Abstract: Visceral leishmaniasis (VL) is a severe chronic systemic zoonotic disease caused by the protozoan Leishmania donovani or L. infantum, in which cytokines play an important role in its pathogenesis. These cytokines are under genetic controls, and their gene polymorphisms have been suggested to exert a functional role in regulating cytokine gene expression. Therefore, the present study determined IFN - γ, IL - 2, IL - 10 and IL - 12 serum levels and gene polymorphisms of 11 cytokine and cytokine receptor genes (IL1A, IL1B, IL2, IL4, IL4R, IL6, IL10, IL12B, TNF, IFNG and TGFB1) in VL patients by ELISA assay and sequence specific primer - PCR method. Gene polymorphism impact on IFN - γ, IL - 2, IL - 10 and IL - 12 serum levels was also evaluated.Forty - four Iraqi Arabs VL patients (age range : 4 months to 12 years) were enrolled in the study. They were referred to hospitals in two Iraqi governorates (Baghdad and Wasit) during the period March 2013 - February 2014. A control sample of 40 apparently healthy individuals was also included and matched patients for age and ethnicity.Most of the ascertained VL cases were in winter and spring with frequencies of 43.2 and 50.3%, respectively. The results also revealed that the age group 1 - 3 years accounted for more than 50% of cases (54.5%) and the infection was more prevalent in male (65.9%) than females (34.1%).A significant increased serum level of IL - 2 (14.72 ± 1.14 vs.4.43 ± 0.49 pg/ml), IL - 10 (40.02 ± 1.26 vs.18.60 ± 1.82 pg/ml), IFN - γ (29.06 ± 1.05 vs. 12.83 ± 1.38 pg/ml) was recorded in VL patients compared to controls. While, IL - 12 serum level showed a non - significant increased level in patients (5.33 ± 3.26 vs.2.17 ± 0.36 pg/ml). Cytokine gene polymorphism analysis revealed that neither genotypes nor alleles of IL1B - 511, IL4R+1902, IL12B - 1188, IFNG+874, TGFB1+896, TNF - 308, IL2 - 330, IL2+166, IL4 - 590, IL4 - 33, IL6nt565/ - 597, IL6 - 174, IL10 - 1082, IL10 - 819 and IL10 - 592 genes showed a significant variation between VL patients and controls. In contrast, a positive association between IL - 1β+3962 CC genotype an C allele and IL4 - 1098 G allele and VL was observed (susceptible genotypes and alleles), while a negative association (protective genotypes and alleles) was recorded for IL1A - 889 TT genotype, IL1B+3962 T allele and IL4 - 1098 TT genotype and T allele. To determine the impact of cytokine genotypes on cytokines serum level, VL patients and controls were distributed according to their serum level in the three genotypes of each cytokine. It was found that TG genotype of IL2 - 330 was observed with the highest IL - 2 level (16.56 ± 1.69 pg/ml) compared to other genotypes. The GT genotype of IL2+166 also showed the highest level of IL - 2 (17.47 ± 2.34 pg/ml) compared to GG (13.53 ± 1.38pg/ml) or TT (15.77 ± 3.88 pg/ml) genotypes in patients. The IL10 - 1082 GG genotype showed the highest level of IL - 10 in patients (45.73 ±3.15 pg/ml) compared to AA (38.02 ± 1.48 pg/ml) genotype. For IL10 - 819 genotypes, they recorded approximated means in patients and no significant difference between them was observed. At the third position of IL10 gene (IL10 - 592), neither patients nor controls demonstrated a significant difference between the means of IL - 10 in their genotypes. At IL12 - 1188, CC genotype showed a significant increase level of IL - 12 (26.16 ±19.76 pg/ml) compared to CA and AA genotype(1.35 ± 0.35 pg/ml and 1.48 ± 0.23 pg/ml respectively) in patients. Finally, neither VL patients nor controls demonstrated a significant difference between the means of IFN - γ in the genotypes of IFNG+874. In conclusion, the role of cytokines in pathogenesis of VL wasascertained, but such role can be better understood in the ground of cytokine gene polymorphisms, which may also have susceptibility or protective effects.

Abstract: حمى التيفوئيد, تسسبب بواسطة بكتريا السالمونيلا تايفي (S. Typhi) Salmonella enterica subspecies enterica serovar Typhi, وينتقل معويا في الانسان حصرا بمدى من التهاب معوي متوسط الى امراض جهازية شديدة وينتشر عن طريق البراز - الفم من خلال تلوث الماء والطعام. هدفت الدراسة الحالية الى تحضير ثلاثة انواع من اللقاحات ضد حمى التيفوئيد من عترة محلية لبكتريا S. Typhi تضمنت لقاح متعدد السكريد Polysaccharide ومتعدد السكريد الشحمي Lipopolysaccharide وخلية كاملة مقتولة Whole cell killed. اجريت هذه الدراسة للفترة من شهر نيسان 2014 وحتى شهر حزيران 2015, تم الحصول على العترة المحلية S. Typhi من المركز الوطني للبحوث والرقابة الدوائية / وزارة الصحة العراقية. شخصت S. Typhi (العترة المحلية) بطرق التشخيص التقليدية كالفحص المجهري والفحوصات البايوكيميائية ونظام التشخيص APi20 وVitek - 2 والفحوصات المصلية, اكدت جميعها هوية البكتريا التي تنتمي الى Salmonella Typhi. تم الحصول على جودة جيدة وفعالية في اللقاحات المحضرة محليا من الناحية البيولوجية والكيميائية والفيزيائية والمناعية مقارنة مع اللقاح التجاري المستورد ، على الرغم من ان لقاحي WCK وLPS رفعا درجة حرارة الجسم في مجموعتي خنازير غينيا الملقحة بهما مقارنة مع المجموعتين المحصنة بلقاحي PSالمحلي والتجاري . ان نتيجة الاختبار الكيميائي (لمتبقيات الفينول والفورمالين) باستخدام تقنيتي Hight - Performance Liquid Chromatography (HPLC) و(Gas Chromatography (GC، قيمة الاس الهيدروجيني PH ومحتوى البروتين امن بالمقارنة مع بروتوكول منظمة الصحة العالمية للقاح التيفوئيد. ايضا في الفحوصات الفيزيائية كان حجم جزيئات لقاح PS المحلي اقل من المستورد، ان صغر حجم جزيئات اللقاح دعم الاستجابة المناعية. في الاختبار المناعي بواسطة تقنية Enzyme Linked Immunsorbent Assay ((ELISA, اظهرت جميع اللقاحات اختلافات معنوية (P<0.05) في مصل خنازير غينيا المعاملة بين الجرعتين الاولى والمعززة مقارنة مع مجموعة السيطرة. كان لقاحي PS التجاري والمحلي ذا اهمية معنوية في التفاعلات المناعية reactogenicity اقل من لقاحي LPS وWCK ، ولكن كل منهم اظهر حماية (70 ، 80 ، 80 و90) ٪ على التوالي ضد جرثومة S. Typhi الضارية. اللقاحات المحلية قد تكون قادرة على تعزيز مناعة التكيفية adaptive immunity من خلال تسجيلها اختلاف معنوي (P<0.05) في حساب مجموع الكريات البيض بين الجرعتين الاولى والمعززة مقارنة مع مجموعتي اللقاح التجاري والسيطرة. واوصت الدراسة تطوير انتاج عراقي للقاح تيفوئيد PS من هذه العترة الضارية. | Typhoid fever, which is caused by Salmonella enterica subspecies enterica serovar Typhi (S. Typhi) is an exclusively human enterically transmitted, range from mild gastroenteritis to severe systemic disease and spread by the fecal - oral route via contaminated food and water. In current study aimed to production three types of vaccines against typhoid fever from local strain S. Typhi, include polysaccharides (PS), Lipopolysaccharide (LPS) and Whole Cell Killed (WCK) vaccine. This study was carried out during the period from April 2014 until June 2015. The local strain of S. Typhi was obtained from The National Center of Drugs Control and Research (NCDCR) / Iraqi Ministry of Health. Identification of S. Typhi (local strain) by traditional methods of diagnosis as microscopic examination and biochemical tests as Api20 system diagnosis, Vitek - 2 and serological test .All confirmed the identity of the bacteria belonging to S. Typhi. Good quality and efficacy was obtained in the locally produced vaccines from the biological, chemical, physical and Immunological compared with the imported commercial vaccine, although WCK and LPS elevated body temperature in a vaccinated guinea pigs groups with them compared with local PS and Commercial Vaccinated groups. That a result of chemical test (phenol and formalin residuals) by High - Performance Liquid Chromatography (HPLC) and Gas Chromatography (GC), PH value and protein content were safety side comparative with WHO protocol typhoid vaccine. Also in physical test, the particles size of local PS vaccine was less than Com. V., the smaller of particles size of vaccine supports the immune response. In Immunological test by Enzyme Linked Immunsorbent Assay ELISA was showed that the titration of all vaccines and have significantly differences (P<0.05) in the sera of treated guinea pigs between first and booster dose compared with control,. The Commercial and local Vi PS. vaccines were significantly less reactogenicity than LPS and WCK vaccines, but all of them provided (70, 80, 80 and 90) % protection respectively against virulent S. Typhi organism. The prepared vaccines may be able to enhance the adaptive immunity through its recorded significant different (P<0.05) in the total leukocytes count between first and booster dose compared with the Commercial Vaccine and control groups. The study recommends to developing production of Iraqi Vi Polysaccharide. Typhoid vaccine from this local virulent strain of S. Typhi.

Abstract: داء الصدفية من الامراض الجلدية الشائعة يتميز بظهور طبقات من الخلايا القرنية المغطاة بقشور سطحية بيضاء اللون ذات بريق فضي كلون الصدفة وتتكون فيه رقع محمرة مع قشور فضية سميكة على الجلد نتيجة انقسام الخلايا المولدة للبشرة بصورة سريعة جدا وهو من الامراض المزمنة غير المعدية ولا تعرف اسباب المرض بصورة محددة ولكن يظهر ان للعوامل المناعية دورا مهما في حدوث المرض ومنها الدور المحتمل للخلل المناعي الذي تم اقتراحه في مرض الصدفية ولاختبار هذه الفرضية اجري فحص المصول لجميع المشاركين لتحري عن حركيات خلوية معينة (IL - 2,TNF - α,IL - 10) والكلوبيولين المناعي ( (IgE وعاملي المتمم ((C3,C4 والعد الكلي والتفريقي لكريات الدم البيضاء (total and diffrentialWBC).شملت الدراسة 55 مريض ( ذكر وانثى ) من مدينة بغداد خلال فترة تشرين ا لثاني 2013 الى نيسان 2014 وتتراوح اعمارهم ما بين (6 - 70 ) سنة وكان 30 منهم غير معالجين و25 معالجين بعلاج بايلوجي ومجاميع سيطرة باجناس وفئات عمرية مماثلة لاغراض المقارنة مكونة من 25 فردا اصحاء ظاهريا.تم فصل المصل قياس تركيز الحركيات الخلوية (IL - 2,TNF - α, IL - 10) والمستوى الكلي للمضاد المناعي (IgE) في مصل الدم المشاركين باستعمال تقنية الامتزاز المناعي مرتبط بالانزيم ELISA)) وعوامل المتمم ((C3,C4 بطريقة الانتشار المناعي الشعاع المنفرد (SRID) وتم قياس العد الكلي والتفريقي لكريات الدم البيض (total and diffrentialWBC).بوساطة جهاز روبي.توصلت الدراسة للنتائج الاتية : 1 - اظهرت النتائج ان متوسط العمر في مرضى الصدفية هو (34.7) سنة وان الفئة العمرية (40 (30 - سنة هي اكثر عرضة للاصابة.2 - اظهر معدل المستوى المصلي للبين البيضاض IL - 10 فروقا معنوية عند مستوى (P<0.05) ما بين مرضى غير المعالجين 17.71 بيكو غرام/ مل ومجموعة السيطرة 1.701 بيكو غرام/ مل. 3 - اظهر معدل المستوى المصلي لعامل التنخر الورمي - الفا فروقا معنوية عند مستوى P<0.05)) ما بين مرضى غير المعالجين 18.6 بيكو غرام/ مل ومجموعة المرضى المعالجين ومجموعة السيطرة13.57 12.85, بيكو غرام/ مل على التوالي .4 - لم تظهر اي فروق معنوية في مع مستوى البين البيضاض IL - 2 عند مستوى P>0.05)) ما بين مرضى غير المعالجين 43.90 بيكو غرام/ مل والاصحاء 43.2 بيكو غرام/ مل.5 - لوحظ وجود ارتفاع معنوي في تركيز المستوى المصلي لبروتين المتمم C3 عند مستوى (P<0.05) في المرضى غير المعالجين138.61 ملغرام/ مل مقارنة بمجموعة السيطرة 37. 123ملغرام / مل . ولقد اظهر بروتين المتمم C4 تركيزا مشابها في مستوياته لكل من العدد الكلي للمرضى ولم يظهر اي فرقا معنويا عند المقارنة مع قيم السيطرة المناظرة لها.6 - اظهرت الدراسة فروقا معنوية في معدل المستوى المصلي للكلوبيولين المناعي IgE الكلي عند مستوى P>0.05)) ,عند المقارنة ما بين مرضى غير المعالجين ومجموعة السيطرة 48.77 ) و29.78 ) وحدة دولية / مل على التوالي .7 - ظهرت زيادة معنوية في معدل كريات الدم البيض ((9.02 % ومعدل الخلايا الوحيدة ((6.71 % والخلايا اللمفية ((27.25 % في المرضى المعالجين بالعلاج البايلوجي مقارنة مع المرضى غير المعالجين والاصحاء.8 - لم تظهر اي فروق معنوية في معدل كريات الدم العدلة والخلايا الحمضة والخلايا الوحيدة بين المرضى غير المعالجين والمعالجين والاصحاء9 - ظهر تاثير الجنس في كلا من العد الكلي لكريات الدم البيضاء والخلايا العدلة والبين الابيضاض 10 حيث وجود فروق زيادة معنويه في الاناث عند مقارنتها مع الذكور.10 - وجود ارتباط ايجابي ومعنوي بين المستوى المصلي للبين ابيضاض IL - 10 وعامل التنخر الورمي - الفا اذ بلغ معامل الارتباط (*r =0.19). | Psoriasis is a chronic skin disease characterized clinical features. It is characterized by sharply demarcated dull - red plaques, covered by thick whitish slivery scales due to the excessive abnormal proliferation of the germenative cells of the epidermis and its immuomediated disease. The disease affect both sexes and genetic of the disease between families. The appearance of psoriasis lesions can be provoked by an external and internal factors.A potential role of the immune dysfunction has been suggested in psoriasis to test this hypothesis, certain cytokines ( IL - 2, IL10,TNF - α), immunoglobulins (IgE),complement components C3 and C4 were investigated in serum ,the total and differential WBC of all participants.The study includes : 55 patients (males and females)from Baghdad City during November 2013 to April 2014 with age range between (6 - 60) years; 30 of them were untreated and 25 were treated with biological treatment (Infliximab) with confirmed diagnosis of psoriasis using standard assessment age and gender matched 25 confirmed healthy control .The serum was separated and cytokines , total immunoglobulinE (IgE) were detected using an enzyme linked immunosorbent assay (ELISA) .The complement factor C3 and C4 were detected using a single radial immunodiffusion (SRID) method and the total and differential WBC was measured by a ruby devise .The study has reached the following results : 1. The age average in patients with psoriasis was (34.7) years and age group (31 - 40) years are more susceptible to affected.2. The study showed that the differences of the serum level of IL - 10 was significant (P<0.05) between untreated psoriasis patients 17.71pg/ml and healthy control 11.70 pg/ml. 3. The study furthermore, has showed the differences of the serum level of TNF - α was significant (P<0.05) between untreated psoriasis patients 18.63pg/ml and both groups treated psoriasis patients and healthy control groups (12.85, 13.57 ,respectively).4. The Research hasn't showed significant differences (P<0.05) serum level of IL - 2 between untreated psoriasis patients 43.90 pg/ml and healthy control43.2 l pg/ml. 5. The results observed a significant increase (P<0.05) in the concentration of serum level of complement component C3 in untreated patients 138.61 mg/dl compared to the control group 37.123 mg/dl .The complement component C4 concentration showed a similar level in each of the total number of patients ,did not show any significant difference when compared with the values of the corresponding control.6. The results have showed , as well , that the rate of serum level for total immunoglobulin IgE significant differences (P<0.05) when comparing between patients 48.7 IU/ml and control group 29.78 IU/ml.7. The Study has Showed significant increase in white blood cells rate 9.02% ,monocyte 6.71% and lymphocytes 27.25% in patients treated with biological treatment (Infliximab) compared with both untreated patients and healthy controls.8. It hasn't showed the significant differences in neutrophil cells and eosinophil ,basophile between healthy controls and both untreated and treated patients .9. There was significant increase in levels of neutrophil ,WBC, IL - 10 in females as compare with males.10. Correlation positive and significant between serum level of IL - 10 and the TNF - α as the correlation coefficient (r = 0.19*).

Abstract: تضمنت الدراسة الحالية جمع 220 عينة من المرضى المصابين بالفطريات الجلديــــــة dermatophytes (63 ذكور و157 اناث) والذين تم تشخيصهم من قبل الكادر الطبي المتخصص في وحدة الامراض الجلدية في مستشفى كلار العام في قضاء كلار التابع لمحافظة السليمانية فی العراق خلال الفترة من منتصف تشرین الثاني 2014 ولغایة حزیران 2015. وقد شملت العینات الجلد المصاب، تقلیم الاظافر واجزاء من الشعر المصاب حيث وضعت العينات في قناني معقمة محكمة الغلق ونقلت الى مختبر الابحاث التابع لقسم علوم الحياة في كلية التربية/ جامعة كرميان تمهیدا لاجراء الفحوصات المختبریة اللازمة لتشخيص الفطريات الجلدية وقد تم التوصل الى النتائج التالية : من مجموع 220 عينة خضعت للفحص المجهري المباشر بواسطة 10% هيدروكسيد البوتاسيوم KOH ، 200 عينة ( 90.9% ) كانت موجبة لفحص هيدروكسيد البوتاسيوم في حين اظهرت نتائج الزرع المختبري نسبة اصابة كان مقدارها 80 عينة موجبة (36.4%). وبالاعتماد على موقع الاصابة بالجسم احتلت سعفة الجسم Tinea corporis اعلی نسبة اصابة بواقع 31% تلتها سعفة القدم Tinea pedis ، سعفة الاظافر، سعفة اليد، سعفة الفخذ ، سعفة الراس وسعفة اللحية حيث تكررت بواقع 26.5% ، 22.5% ، 8.5% ، 6% ، 5% و0.5% على التوالي. كشف الزرع المختبري عن نمو 80 عزلة من الفطريات الجلدية والتي تعود الى ثلاثة اجناس Trichophyton، Microsporum وEpidermophyton و13 نوع وكان الفطرTrichophyton rubrum هو الاكثر انتشارا وبنسبة 35% من بين الفطريات الجلدية التي تم عزلها وقد عزل النويع Trichophyton rubrum var.raubitschekii لاول مرة في العراق حيث تم عزله من سعفة الفخذ Tinea cruris. الفطر T.mentagrophytes سجل نسبة اصابة 17.5% من الفطريات الجلدية المعزولة تلاه الفطــر M.canis بنسبة 10% وT.tonsurans بنسبة 7.5% . اما الفطريات الجلدية الاخرى والتي شملت T. soudanese; T.interdigitale; T. terrestre فقد تكررت بنسبة 5% ، بينما T.concentricum; T. schoenleinii; T.verucosum; MicrosporumM. audouinii; M.gypsium وE.floccosum فقد تكررت وبواقع 2.5% لكل منها. اوضحت نتائج هضم الجين ITSالواقع في rDNA والتي تم مضاعفتها باستخدام تقنيةPCR عن التعدد الشكلي لطول للجين مابين الانواع المختلفة للفطريات الجلدية المعزولة. حيث اظهرت نتائج هضم الجين ITS لثمانين عزلة من الفطريات الجلدية والتي تعود الى 13 نوع عدد من الحزم المختلفة باستثناء الفطر M.auouiniiوالذي يفتقد الى موقع القطع الذي يعمل عليه انزيم القطع BstN1. ساعدت الدراسة الحالية على رسم العلاقة التطورية ما بين الانواع المختلفة للفطريات الجلدية المعزولة من خلال فحص التسلسل او التتابع الجينـــــــــي Gene sequencing للجين ITS,5.8S,ITS2، وقد حللت نتائج باستخدام برنامج تسلسل قاعدة البيانات Robust Phylogenetic Analysis program لرسم شجرة التقارب الجيني مابين الانواع الفطرية. تم الحصول على 12 سلالة لبكتريا Lactobacillus تعود لتسعة انواع وهيLactobacillus pentosus; Lactobacillus plantarum 1; Lactobacillus plantarum 2; Lactobacillus brevis 1; Lactobacillus brevis 3; Lactobacillus acidophilus; Lactobacillus collinoides A,B ; Lactobacillus fermentum; Lactobacillus helveticous ; Lactobacillus paracasei وLactobacillus salivarious . اظهرت دراسة الفعالية التثبيطية لانواع وسلالات بكتریا Lactobacillus على الفطرالاكثر تكراراTrichophyton rubrum باستخدام الراشح البكتيري ان النوع L.collinoides A هو الاقوى تاثيرا حيث وصل قطر منطقة التثبيط الى 14mmقبل اضافة الكليسرول ، غير ان النتائج التي تم الحصول عليها بعد اضافة الكليسرول اظهرت فرق كبير في فعالية البكتريا حيث وصل قطر منطقة التثبيط باستخدام النوع ذاته الى 33mm اما قيم MIC وMFC باستخدام طريقة التخفيف فقد بلغت 62.5) و(125 µL\ml على التوالي. اقوى الانواع البكتيرية التي اظهرت فعالية تضادية ضد الفطرT.rubrum باستخدام الخلايا الكاملة شملت L. brevis 3; L. pentosus; L. acidophilus; L.plantarum والتي سببت تثبيط كامل لنمو الفطر كما ان قيم MIC وMFC باستخدام الخلایا الكاملة فقد بلـــــــغت (15.6 و31.3) µL\ml للانواع الـــتالية L. brevis 3; L. pentosus وL.collinoidis A على التوالي ، اما الانواع البكتيرية الطبيعية التي تم الحصول عليها من المهبل وهي L.plantarum 1 وL.acidophilus فقد اظهرت القيم التالية 31.3) وµL\ml (62.5 من MIC وMFC على التوالي. اظهرت نتائج التشخيص باستعمال جهازكروماتوغرافيا السائل عالي الكفاءة HPLC لقیاس تركیز احد مضادات الفطريات وهو Phenylacetic acid (PLA) في الراشح البكتيري لجميع السلالات البكتيرية والتي عددها 12 وتعود لتسعة انواع لبكتريا Lactobacillus بالاضافة الى الكبسولة العلاجية المستخدمة كسيطرة موجبة ان النوع L.plantarum 1 انتج اعلى تركيز من PLA الذي بلغ 19.6 ppmوالذي فاق تركيزه في السيطرة الموجبة البالغ 17.9ppm تلاه L.brevis 3 ثم L.acidophilus حيث بلغ تركيز PLA 17.2ppm و16.4 ppm على التوالي. | Two hundred and Twenty patients (157 females and 63 males) were examined and suspected to be infected with dermatophytes under the supervision of specialized physicians at the Dermatology unit of the General Hospital in Sulaimania Province\Iraq during the period from the middle of November 2014 to June 2015. Specimens were collected from patients included hair fragments, skin scraping and nail clipping which transferred to the research laboratory of Biology Department at College of Education\ University of Garmian, where they immediately examined. Out of 220 clinical specimens were subjected to the direct microscopic examination using 10% KOH, 200(90.9%) specimens were showed positive KOH test, while the laboratory culturing obtained 80(36.4%) positive cultures of dermatophytes. According to the infected sites of the body, tinea corporis was the most common type (31%) followed by tinea pedis, tinea unguium, tinea manuum, tinea cruris, tinea capitis and tinea barbae which were recorded 26.5%, 22.5%, 8.5%, 6%, 5% and 0.5% respectively. Based on the conventional laboratory methods, 80 clinical isolates of dermatophytes showed positive culture which belonging to three genera Trichophyton, Microsporum and Epidermophyton and 13 species, Trichophyton rubrum was the most common species (35%) and Trichophyton.rubrum var.raubitschekii was isolated from tinea cruris for the first time in Iraq. Trichophyton mentagrophytes constitute 17.5% from the isolated dermatophytes followed by Microsporum canis (10%) and Trichophyton tonsurans (7.5%). Other species Trichophyton soudanese; Trichophyton interdigitale; Trichophyton terrestre represented 5% of dermatophytes while Trichophyton concentricum; Trichophyton schoenleinii; Trichophyton verucosum; Microsporum audouinii; Microsporum gypseum and Epidermophyton floccosum constitute only 2.5% of dermatophyte species.The results of PCR - RFLP showed the polymorphism of the Internal Transcript Spacer (ITS) region of the rDNA gene for different species of isolated dermatophytes. The results of the digested ITS PCR products of 80 clinical isolates belonging to 13 species revealed a number of recognition patterns with the exception of Microsporum audouinii which lacked restriction site of the used endonuclease restriction enzyme BstN1. The current study helped to drawing the phylogenetic relationship between the isolated dermatophytes by sequencing of the ITS1, 5.8S, and ITS2 gene. The ITS database sequences introduced Robust Phylogenetic Analysis program to draw the phylogenetic tree between dermatophyte species. Twelve strains belonging to nine species of Lactobacillus were identified as Lactobacillus pentosus; Lactobacillus plantarum 1; Lactobacillus plantarum 2; Lactobacillus acidophilus 1; two strains of Lactobacillus collinoides (identified as A and B); Lactobacillus fermentum 1; Lactobacillus helviticous, Lactobacillus brevis 1; Lactobacillus brevis 3; Lactobacillus paracasi and Lactobacillus salivarious. The study of antagonistic effect of Lactobacilli species and strains against the most frequent species of dermatophytres Trichophyton rubrum using the cell free supernatant (CFS) showed that L. collinoides A recorded the highest inhibition zone reached to 14 mm before addition of glycerol and increased to 33 mm after supplying glycerol into the MRS growth culture for the same species. The results of MIC and MFC values of the CFS were demonstrated by microdilution assay reached to (62.5 and 125) µL\ml respectively.The strongest species of Lactobacillus showed antifungal activity against T.rubrum by using entire cells included L. brevis 3; L. pentosus; L. acidophilus and L. plantarum were completely inhibited the fungal growth and the results of MIC and MFC of the bacterial entire cells reached to the values (15.6 and 31.3) µL\ml respectively for L. brevis 3; L. pentosus and L. collinoides A, in addition to the positive control while vaginal microbiota L. plantarum 1 and L. acidophilus exhibited (31. and 62.5) µL\ml respectively. The results of High Performance Liquid Chromotography (HPLC) analysis of antifungal compound, Phenylacetic acid (PLA) in the fermented CFS of 12 strains belonging to 9 species of Lactobacillus in addition to the probiotic capsule showed that the highest value of PLA was produced by L. plantarum 1(19.6 ppm) which is higher than the positive control (17.9 ppm) followed by L. brevis 3 and L. acidophilus reaching concentration of PLA 17.2 and 16.4 ppm respectively

Abstract: داء الذئب الاحمراري هو احد امراض المناعة الذاتية ولم يتم معرفة المسببات جيدا على وجه الدقة، ولكن تم اقتراح الاستعداد الوراثي والعوامل البيئية، فضلا عن العوامل الهرمونية لما لها من دور مؤثر في امراضية داء الذئب الاحمراري. حددت هذه الدراسة العلاقة الوظيفية بين مستوى مصل خمسة من السايتوكينات (IL - 2، IL - 4 وIL - 6، IL - 10 وIL - 12) وتعدد الاشكال الخلوية النيوكليوتيدية المنفردة بها؛ تعدد الاشكال في 11 موقع (IL2 - 330، IL2 + 160، IL4 - 1098، IL4 - 590، IL4 - 330، IL6 - 174، IL6 + 565، IL10 - 1082، IL10 - 819، IL10 - 592 وIL12B - 1188) في عينة من 45 من النساء العراقيات العربيات المصابات بداء الذئب الاحمراري من اللواتي يراجعن العيادة الاستشارية/ قسم امراض الروماتيزم في مستشفى بغداد التعليمي خلال الفترة من حزيران ولغاية تشرين الاول 2013. فضلا عن، شملت الدراسة 43 من النساء الاصحاء كمجموعة سيطرة. وتم الحصول على النتائج التالية : كان هنالك انخفاض كبير في المكونات المتممة C3 وC4 (P ≤ 0.001) لدى مرضى داء الذئب الاحمراري (0.57 ± 0.04 و0.06 ± 0.01 غم/ لتر على التوالي) مقارنة مع مجموعة السيطرة (1.12 ± 0.04 و0.22 ± 0.01، غم/ لتر، على التوالي). وكان معظم المرضى ايجابيين للمضاد النووي والاجسام المضادة لشريط الحامض النووي منقوص الاوكسجين المزدوج المضاد (91.1 و88.9٪ على التوالي)، في حين كانت هنالك نسبة 9.3٪ فقط من مجموعة السيطرة الايجابيين لهذه الاجسام المضادة وكان هذا الفرق كبير (P ≤ 0.001) في كلتا الحالتين. واظهرت عمليات تقييم مستويات السايتوكينات في المصل ان IL - 2 ينخفض انخفاض معنوي كبير(P ≤ 0.001) لدى مرضى داء الذئب الاحمراري مقارنة بمجموعة السيطرة (25.6 ± 1.2 مقابل 35.1 ± 2.1 بيكوغرام/ مل)، وكان هنالك انخفاض في الانترلوكين - 4 ايضا لدى المرضى ( 17.9 ± 2.7 مقابل 22.2 ± 1.9 بيكوغرام / مل)، ولكن هذا الانخفاض لم يكن معنويا (P > 0.05). مقابل16.7 ± 1.0 بيكوغرام/ مل) 26.0 ± 4.3) وعلى العكس فان الانترلوكين - 6 والانترلوكين - 10 36.4 ± 1.8) مقابل 29.5 ± 0.8 بيكوغرام/ مل)، والانترلوكين - 12 24.4 ± 2.3) مقابل 19.2 ± 0.9 بيكوغرام/ مل) اظهرت زيادة كبيرة لدى مرضى داء الذئب الاحمراري.تم تشخيص 11 من السايتوكينات المتعددة الاشكال الخلوية المنفردة واظهر فقط الموقع 590 في الانترلوكين - 4 والموقع 188 في الانترلوكين - 12 اختلافات كبيرة بين المرضى ومجموعة السيطرة. وقد لوحظ ان هناك زيادة معنوية في تردد الطراز الجيني TTفي الموقع 590 للانترلوكين - 4 (66.6 مقابل 41.9٪، ونسبة الخطا النسبي = 2.78؛ P = 0.03) والتي تم ملاحظتها في المرضى. في المقابل، انخفض تردد الاليل C بشكل ملحوظ لدى المرضى (24.4 مقابل 41.9٪، PF = 0.23؛ P = 0.02) والاليل T (P = 0.02 75.6 مقابل 58.1٪، ونسبة الخطا النسبي = 2.23). وبالنسبة للموقع B - 1188في الانترلوكين - 12، اظهر الطراز الوراثي CC زيادة معنوية في التردد لدى المرضى مقارنة مع مجموعة السيطرة (73.4 مقابل 46.5٪، ونسبة الخطا النسبي في = 3.16؛ P = 0.016)، ولوحظت زيادة مماثلة في التردد في الاليل C (80.0 مقابل 62.8٪ ؛ RR = 2.37؛ P = 0.013). وعلى العكس، فان الطراز الوراثي AC (13.3 مقابل 32.6٪، PF = 0.22؛ P = 0.042) والليل A (20.0 vs.37.2٪، PF = 0.22؛ P = 0.013) انخفضت تردداتها بشكل ملحوظ لدى المرضى. وجرى تقييم اثر تشخيص الطراز الوراثي للاشكال الخلوية المتعددة المنفردة للسايتوكينات على مستوى المصل لكل من الانترلوكين - 2، الانترلوكين - 4 والانترلوكين - 6 ، الانترلوكين - 10 والانترلوكين - 12 لدى مرضى داء الذئب الاحمراري ومجموعة السيطرة. وتم الحصول على ملاحظات مختلفة، وارتبطت بعض الانماط الجينية مع زيادة مستوى السايتوكينات، في حين ارتبطت الاخرى مع انخفاض المستوى. في كلتا الحالتين لوحظت اختلافات بين مرضى داء الذئب الاحمراري ومجموعة السيطرة. | Systemic lupus erythematosus (SLE) is an autoimmune disease and its precise etiology is not well - defined, but genetic predisposition and environmental factors, as well as hormonal factors have been suggested to play important roles in SLE pathogenesis. The present study determined the functional correlation between serum level of five cytokines (IL - 2, IL - 4, IL - 6, IL - 10 and IL - 12) and their single nucleotide polymorphisms; SNPs at 11 positions (IL2 - 330, IL2+160, IL4 - 1098, IL4 - 590, IL4 - 330, IL6 - 174, IL6+565, IL10 - 1082, IL10 - 819, IL10 - 592 and IL12B - 1188) in a sample of 45 Iraqi Arab female SLE patients, who were referred to the Consultant Clinic at the Department of Rheumatology, Baghdad Teaching Hospital during the period June - October 2013. In addition, 43 apparently healthy women were further included in the study as a control group. The following results were obtained : Complement components C3 and C4 were significantly (P ≤ 0.001) decreased in SLE patients (0.57 ± 0.04 and 0.06 ± 0.01 g/L, respectively) compared to controls (1.12 ± 0.04 and 0.22 ± 0.01, g/L, respectively).Most of the patients were positive for anti - nuclear and anti - double strand DNA antibodies (91.1 and 88.9%, respectively), while only 9.3% of controls were positive for these autoantibodies. Such difference was significant (P ≤ 0.001) in both cases.Assessments of cytokine serum levels revealed that IL - 2 showed a significant (P ≤ 0.001) decreased level in SLE patients compared to controls (25.6 ± 1.2 vs. 35.1 ± 2.1 pg/ml), and IL - 4 was also decreased in patients (17.9 ± 2.7 vs. 22.2 ± 1.9 pg/ml), but the difference was not significant (P > 0.05). In contrast, IL - 6 (26.0 ± 4.3 vs. 16.7 ± 1.0 pg/ml), IL - 10 (36.4 ± 1.8 vs. 29.5 ± 0.8 pg/ml) and IL - 12 (24.4 ± 2.3 vs. 19.2 ± 0.9 pg/ml) were significantly increased in SLE patients.Out of the 11 investigated cytokine SNPs, only IL4 - 590 and IL12B - 1188 showed significant variations between patients and controls. A significant increased frequency of IL4 - 590 TT genotype (66.6 vs. 41.9%; Odd’s ratio = 2.78; P = 0.03) and T allele (75.6 vs. 58.1%; Odd’s ratio = 2.23; P = 0.02) was observed in patients. In contrast, C allele frequency was significantly decreased in patients (24.4 vs. 41.9%; PF = 0.23; P = 0.02). For IL12B - 1188, CC genotype showed a significant increased frequency in patients compared to controls (73.4 vs. 46.5%; Odd’s ratio = 3.16; P = 0.016), and a similar increased frequency was observed in C allele (80.0 vs. 62.8%; RR = 2.37; P = 0.013). In contrast, AC genotype (13.3 vs. 32.6%; PF = 0.22; P = 0.042) and A allele (20.0 vs.37.2%; PF = 0.22; P = 0.013) frequencies were significantly decreased in patients.The impact of investigated cytokine SNP genotypes on serum level of IL - 2, IL - 4, IL - 6, IL - 10 and IL - 12 was also assessed in SLE patients and controls. Different observations were made and some genotypes were associated with increased level of cytokines, while others were presented with a decreased level. In both cases differences between SLE patients and controls were observed.

Abstract: ﺷﻤﻠﺖ ﻫﺬﻩ ﺍﻟﺪﺭﺍﺳﺔ جمع ( ٢٢۰ ) عينة من Hospitalized Patients في مستشفى بغداد التعليمي في مدينة الطب (العراق/ بغداد)، خلال الفترة من شهر تشرين الثاني /٢۰۱٣ الى شهرنيسان/ ٢۰١٤. تضمنت العينات )۷۰) مسحة من الحروق ، (٥٦) عينة دم،(٥٤) مسحة من الاذن، (٤۰) عينة ادرار. ﺍﻅﻬﺮﺕ ﺍﻟﺩﺭﺍﺳﺔ ﺍﻥ۱۰۸) ٤۹٫۰٣٪ (من العزلات تعود الى جنس Staphylococcus aureus حيث اعطت نتيجة موجبة لفحص الانزيم المخثر للبلازما (Coagulase - Positive Staphylococci) COPS،في حين اعطت ۱۱٢ )٥۰٫۹٪(من العزلات نتيجة سالبة لفحص الانزيم المخثر للبلازماCoagulase - Negative Staphylococci )CONS .من بين ۱۰۸ عزلة بكتيرية من COPS شخصت۸٥ )۷۸٫۷٪( عزلة بكتيرية على انها بكتريا العنقوديات الذهبية المقاومة للميثسيلين MRSA ) ( methicillin resistance Staphylococcus aureus طبقا لاختبار الحساسية للمضادات الحياتية وجهاز الفايتكvitek system،بينما شخصت ٢٣ )٢۱٫۹٪( عزلة بكتيرية على انها بكتريا العنقوديات الذهبية الحساسة للميثسيلين methicillin sensitive Staphylococcus aureus) MSSA . تم تحديد اختبار الحساسية لعزلات المكورات العنقودية لعشرة من مضادات الحياة باستخدام طريقة الانتشار بالاقراص،وكانت نسبة مقاومة بكتريا المكورات العنقودية للمضادات كالاتي : جنتامايسين ٦۰ )٥٥٫٥٦٪( ,ارثرومايسين ۷۰)٦٤٫۸١٪(،كلندامايسين ۸١)۷٥٪( ,سيفوكستين ۸۹)۸٢٫٤١٪( ,كلورامفينيكول وبنسيلين جي ۹۰)۸٣٫٣٣٪( ,اوكزاسلين ۸٦)۷۹٫٦٣٪(،فانكومايسين ٢٤ )٢٢٫٢٢٪(،واخيرا الميثسيلين ۸٥)۷۸٫۷۰٪(.ﻭﻟﻘﺪ اﻅﻬﺮﺕ ﺍﻟﻨﺘﺎﺋﺞ ﺍﻟﺘﻲ ﺗﻢ ﺍﻟﺤﺼﻮﻝ ﻋﻠﻴﻬﺎ ﻣﻦ ﺍﺧﺘﺒﺎﺭ ﺍﻟﺤﺴﺎﺳﻴﺔ ﻟﻠﻤﻀﺎﺩﺍﺕ ﺍﻟﺤﻴﻮﻳﺔ انها ﻛﺎﻧﺖ ﻣﻘﺎﻭﻣﺔ ﻟﻠﻌﺪﻳﺪ ﻣﻦ ﺍﻟﻤﻀﺎﺩﺍﺕ ﺍﻟﺤﻴﻮﻳﺔ ﺍﻷﺧﺮﻯ ﺑﺎﻹﺿﺎﻓﺔ اﻟﻰ للميثسيلين. نتائج جهاز الفايتك كانت متقاربة مع نتائج اختبار الحساسية للمضادات الحياتية لنفس المضادات التي استخدمت في كلا الاختبارين ،ماعدا نتيجة مضاد الفانكومايسين حيث كان الفرق بالنتيجة كبير ،كانت نسبة الحساسية للمضاد بواسطة الانتشار بالاقراص ٢٤ )٢٢٫٢٢٪(، بينما بواسطة جهاز الفايتك كانت النتيجة )۸٫٣٣٪(.السبب وراء الاختلاف بالنتيجة قد يعزى الى ظروف الاختبار ونوعية التقنية المستخدمة. اعلى نسبة من المكورات العنقودية الذهبية المقاومة للميثسيلين عزلت من عينات الجروح ،حيث بلغت هذه النسبة (۹٣٫۸۷٪)،في حين اقل نسبة من هذه االبكتريا عزلت من عينات الادرار ،حيث بلغت النسبة (٥۰٪). اجري تشخيص تاكيدي للعزلات باستخدام تقنية تفاعل سلسلة البلمرة لمعرفة عائديتها الى نفس النوع من البكتريا عن طريق تضخيم جينmecA ،واظهرت ۸٥ عزلة بكتيرية نتيجة موجبة تفيد باحتوائها على هذا الجين. في الدراسة الحالية، مضاد السفتارولاين هو المضاد المثالي للعلاج، بسبب فعاليته القاتلة ضد بكتريا العنقوديات الذهبية المقاومة للميثسيلين .حيث اختبرت فعاليتة وتاثيره عللى ۸٥ عزلة من البكتريا المقاومة للميثسيلين،كل العزلات التي اختبرت كانت حساسة لهذا المضاد.تم تحديد اختبار التركيز المثبط الادنى MIC لعزلات المكورات العنقودية الذهبية لمضاد السفتارولاين,۸١( ۹٥٫٢۹٪) عزلة كانت قيمة ال MIC لها ۰٫٥مايكروغرام/مل ، ٣ (٣٫٥٢٪) ثلاث عزلات كانت قيمة ال MIC لها (۰٫٢٥مايكروغرام/مل) , و۱(۱٫۱۷٪) عزلة واحدة كانت قيمة ال MIC لها (۰٫١٢ مايكروغرام/مل) . اظهرت النتائج ان قيم تركيز المثبط الادنى للمضاد قد تراوحت بين (۰٫٥ - ۰٫١٢ مايكروغرام/مل). | Two hundred and twenty clinical samples were collected from hospitalized patients in Baghdad Teaching Hospitals in Medical City (Baghdad /Iraq), during the period From November/ 2013 to April /2014. The samples included : (70) burn swab, (56) blood, (54) ear swab, and (40) urine sample. It was found that (49.03%) of the 108 isolates, which belong to Staphylococcus aureus, were diagnosed as coagulase - positive staphylococci (COPS); while 112 (50.9%) of clinical samples were coagulase negative (CONS). From the collected clinical samples that gave positive result in coagulase (108) there 85(78.7%) of it was (MRSA) methicillin resistance S. aureus according to sensitivity test and vitek 2 system. The rest were (MSSA) methicillin susceptible S. aureus 23(21.9%). The pattern of antibiotic susceptibility of S. aureus isolates included 10 antibotics (Gentamycin, Erythromycin Clindamycin, Cefoxitin, Chloramphenicol, Penicillin G, Oxacillin, Vancomycin, Tetracycline, and Methicillin), which determined using disc diffusion method. There were a variable resistance percentage for the rest of antibiotics : Gentamycin 60 (55.56%), Erythromycin 70(64.81%), Clindamycin 81 (75.00%), Cefoxitin 89 (82.41%), Chloramphenicol and Penicillin G, 90 (83.33%), Oxacillin 86 (79.63%), Vancomycin 24 (22.22%), and Methicillin 85 (78.70%) . The results obtained from antimicrobial sensitivity test showed that MRSA isolates were resistant to many other antimicrobial agents in addition to the Methicillin (Multi - drug Resistant). The results of vitak - 2 were closely with those obtained from sensitivity test for all antibiotics that used in both tests, except vancomycin; the percentage of vancomycin in disk diffusion was 24 (22.22%), while the percentage in vitek system was 9 (8.33%).The reason behind the difference in results is attributed to the conditions of tests used and type of techniques. The highest percentage of MRSA isolate was from burn samples; it was 93.87%, while the lower percentage of MRSA isolate was from urine samples (50%). A confirmatory test was carried out for the selected isolates using PCR technique for further characterization up to the species level by the amplification of mecA gene (Staphylococcus aureus specific gene) and all the isolates are found to be positive for the presence of mecA gene. In the present study, ceftaroline is a drug of choice, because of its bactericidal activity against Methicillin - resistant S. aureus .The effectiveness of the ceftaroline antibiotic is being tested on 85 isolates of Methicillin - resistant S. aureus (MRSA); all isolates were sensitive to ceftaroline. The pattern of minimum inhibitory concentration of MRSA isolates to ceftaroline, was determined using MIC method. For 81 (95.29%) MRSA isolates the MIC (0.5 µg/ml), 3 (3.52%) MRSA isolates the MIC (0.25 µg/ml), and 1(1.17%) MRSA isolate the MIC (0.12 µg/ml). The results revealed that MIC range from (0.5 µg/ml - 0.12 µg/ml) respectively.

دراسة الميكروبية والتداخل البكتيري للفطريات المصاحبة لالتهابات الانف والاذن لمرضى السكري وابيضاض الدم == Microbial Study and Bacterial Interaction with Mycosis Associated with Nose and Ear Inflammatory of Diabetes and Leukemia Patients

Abstract: One hundred and ten clinical samples were collected from hospitalized patients in hematology - oncology department in Baghdad Teaching Hospital and Al - Kindy Teaching Hospital, during the period from November 2014 to June 2015.The samples included 48 female and 62 male patients who are suffering from diabetes mellitus (DM) and leukemia. The ages of patients ranged between 13 - 82 years. The swab samples taken from the nose and ear of patients were (67 with diabetes mellitus and 43 with leukemia). The results of fungal culturing revealed that 50 samples of DM patients (74.63%) and 31 samples of leukemia (72.09%) were diagnosed as positive ones.From the collected clinical samples that gave positive results; it was found that Candida spp. was the most common fungi isolated from ear swabs (50%) followed by Aspergillus spp. (42.86%) and Penicillum spp. (7.14%).The results of fungal (molds) species isolated from the nose of DM patients revealed highly significant differences between fungal rates, Aspergillus spp. were isolated with highest levels (48.15%), followed by Penicillum spp. (37.04%); however, Rhizopus spp. was isolated with low levels (14.81%). On the other hand, it was found that Candida spp. and Rhodotorula spp. were the most yeasts isolated from the nose of DM patients. They were isolated with rates as (45.83%) and (41.67%) respectively. Nevertheless, the genus Cryptococcus spp. was isolated with low levels (12.50%). The results also revealed that males were more susceptible to fungal infection compared to females. Moreover, it was found that the fungal are more prevalent in ages over fifty and ages between thirty one and fifty years compared with younger ages between ten and thirty years old. From the collected clinical samples of leukemia patients, highly significant differences were found between the rates of fungal infection. The level of Aspergillus spp. was (90.91%) and Rhizopus spp. (9.09%), while the levels of yeasts were (88.24%) and (11.76%) for Cryptococcus spp. and Candida spp. respectively.The frequent species of Aspergillus identified included A. niger (37.78%), followed by A. flavus (35.56%), A. fumigatus (17.77%) and A. terreus with lowest levels (8.89%). The results also revealed that males are more susceptible to fungal infection compared to females. Moreover, it was found that the fungi are more prevalent in ages 31 - 50, followed by younger ages (10 - 30) compared to old ages, over fifty years old.All fungal isolates were identified according to macroscopic and microscopic characteristics in addition to biochemical tests for yeast identification. Vitek 2 system is another confirmatory test which was carried out for further characterization for the yeast species. The results of Vitek 2 system revealed that Candida spp. were isolated with the highest level (41.67%), followed by Cryptococcus spp. (37.5%) however, Rhodotorula spp. was isolated with a low level (20.83%). A confirmatory test for the selected isolates of A. fumigatus using polymerase chain reaction (PCR) analysis technique was also conducted. This test has been done by using primers specific for hemolysin gene (aspHS). The results of the molecular method were closely related to those obtained from cultural methods with exception of one isolate which was positive by PCR method nevertheless, it was negative by culture method. The results of PCR analysis technique, revealed that 6 isolates out of total 8 (75%) were positive for the presence of aspHS gene compared to culture methods which showed 5 isolates out of total 8 (62.5%) had the ability to induce hemolysis when grown on blood agar .The inhibitory effect of Bacillus subtilis against Aspergillus spp. was also conducted, the results indicated that B. subtilis isolate has antimicrobial activity against Aspergillus spp; the diameter of inhibition fungal growth were 39.3 ± 01.5, 48.6 ± 02.2, 40.5 ± 02.8 and 43.5 ± 01.7 mm for A. fumigates, A. flavus, A. niger and A. terreus respectively compared with the diameter of controls which were 80.0 ± 05.3, 78.0 ± 03.9, 43.5 ± 01.7 and 72. ± 02.6 mm respectively. The radial inhibition growth of Aspergillus spp. treated with B. subtilis were 51%, 38%, 47% and 44% for A. fumigatus, A. flavus, A. niger and A. terreus respectively compared with treatment of control.

Abstract: قد تم تصميم هذه الدراسة لتسليط الضوء على دور بعض الحركيات الخلويه ( IL - 13, IL - 18) والكلوبيولين المناعي الكلي ((T - IgE والكلوبيولين النوعي(S IgE) ل 20 مسبب للحساسيه داخلي وخارجي واعداد خلايا الحمضات. وكان مجموع مرضى الحساسية 168، [99 مريضا بالربوالتحسسي (43 ذكرا)، (56 انثى)]، [69 مريض بالتهاب الجيوب الانفيه التحسسي (29 ذكرا)، (40 انثى)]، وجمعت اربعين عينه لاشخاص اصحاء [ (15 ذكور)، (25 اناثا )]. الفئة العمرية (5 - 60) سنة تم جمعها من المركز التخصصي للحساسية في بغداد / الرصافة وخلال الفترة من تشرين الثاني 2016 الى شباط 2017. تمت دراسه بعض التغيرات الدمويه ومنها قياس نسبه ال (Hb) والعدد الكلي لخلايا ال (WBC) واعداد خلايا الحمضه لجميع المرضى والاصحاء عن طريق استخدام محلل الالي (Beckman Coulter) والعده الخاصه بالجهاز . تم قياس الاختبار المناعي (IL - 13, IL - 18, T - IgE) لجميع المرضى والاصحاء باستخدام جهاز (ELISA ) والعدد الخاصه بالاختبارات. تم قياس الكلوبيولين النوعي (Spesific IgE) الخاص بالمحسسات المستنشقه (عث الفراش، الحيوانات الاليفه ,الصرصر، الاعفان، حبوب اللقاح الاعشاب، حبوب اللقاح الاشجار ,حبوب اللقاح الحشائش) لجميع المرضى والاصحاء باستخدام تقنيه (Western Immunobloting Electrophoresis test) والعده الخاصه بالجهاز والفحص (Euro line Inhalation Profile Kits).توصلت هذه الدراسه الى النتائج التاليه : 1 - عدم وجود فروق معنويه بين المجاميع المدروسه طبقا للجنس . 2 - عدم وجود فروق معنويه بين مرضى الربوالتحسسي الذين لهم تاريخ عائلي ايجابي وسلبي. بينما كانت هناك فروق معنويه بين مرضى التهاب الجيوب الانفيه التحسسي الذين لديهم تاريخ عائلي سلبي وايجابي .3 - احصائيا، كانت الفئة العمرية و(21 - 40) الاكثر شيوعا في مرضى الربو التحسسي والتهاب الجيوب الانفيه التحسسي حيث بلغت نسبتها المئويه على التوالي[63، 63.6٪)، (47،68.1٪)].4 - زيادة معنوية في النسبه المئويه لاعداد الحمضات (٪) ومستوى الكلوبيولين المناعي الكلي ( T - IgE) ظمن مجموعتي الحساسية مقارنتا بمجموعه السيطره.5 - اظهرت النتائج وجود فروق معنويه بين اعداد المرضى الذين لديهم الكلوبيولين المناعي الخاص بالمحسسات ((Spesific IgE واعداد المرضى ذوي النتيجه السالبه لدى مرضى الربو التحسسي والتهاب الجيوب الانفيه التحسسي . 6 - سجل محسس الصراصر اعلى نسبة محسس لدى مرضى الربوالتحسسي وتلتها مسببات الحساسية للحيوانات الاليفة وحساسية حبوب اللقاح. بينما في مجموعة التهاب الجيوب الانفيه التحسسي اعلى نسبة محسسات كانت للحيوانات الاليفة وتلتها حبوب لقاح الحشائش .7 - زيادة معنويه كبيرة في مستوى الحركيات الخلويه (IL - 13, IL - 18) عند مرضى الربو التحسسي والتهاب الجيوب الانفيه التحسسي .8 - وجود علاقة ارتباط معنوية كبيرة بين النسبه المئويه لاعداد خلايا الحمضات والكلوبيولين المناعي الكلي( T - IgE) في مرضى الربو التحسسي والتهاب الجيوب الانفيه التحسسي r = 0.424)، (P = 0.00 | This study is designed to shed light on the role of IL - 13, IL - 18, T - IgE, Spesific IgE for 20 Indoor and outdoor inhalant allergens in some of type - 1 allergic disease and Eosinophil cells count . The Total allergic patients have been 168, 99 asthmatic patients (43 male ), (56 female ), 69 Rhinitic patients (29 male ), (40 female) and Apparently Healthy Control AHC 40 (15 male ) , (25 female ). Their age group (5 - 60) years . The groups have been collected from the Specialized Center Of Allergy in Baghdad / AL - Resafa and during the period from October 2016toFebruary2017. Some Hematological changes have been studied, including the measurement the Hb% , total count WBC, and eosinophil cells % for all subject by using automated analyzer (Beckman Coulter Analyzer). Beside some Immunological changes were studies including (IL - 13, IL - 18 and total IgE) which have been measured for all subjects by using Enzyme Linked Immunosorbent Assay (ELISA) . The measurement of serum specific IgE for some inhalant allergens (HDM, Pits, Cockroach, Mites, Grasses pollen, Tree pollen and Weed pollen ) allergens for all subject by using Western Immunobloting test. The Study has reached the fallowing results.1 - There are no significant differences between the studied groups according to gender.2 - There are significant differences in family history between rhinitis group and control. 3 - Statistically, the age group (21 - 40) are most frequent within allergic asthma and rhinitis are (63, 63.6%), (47,68.1%) respectively. 4 - Highly significant increase in Eosinophil count (%) and total IgE within allergic groups when compared with control. 5 - Highly significant differences between positive specific IgE inhalant allergens (positive) and negative patients in asthma and rhinitis groups. 6 - The cockroach allergen records the highest percentage in asthmatic patients fallowed by pets allergens and grasses pollen allergens. While in rhinitic group the highest percentage is for pets allergens fallowed by grasses pollen , cockroach allergen. 7 - Highly significant increase in IL - 13 and IL - 18 level in asthmatic and rhinitic patients. 8 - Highly significant correlation between eosinophil cells count (%) and total IgE In asthmatic and rhinitic patients (r = 0.424 ), (P=0.00).

Abstract: هدف البحث الى التعرف على فاعلية التدريس باستراتيجية التعلم بالتعاقد في تحصيل مادة علم الاحياء لدى طلاب الصف الثاني المتوسط وتفكيرهم الابداعي. وللتحقق من ذلك تم صياغة الفرضيتين الصفريتين الاتيتين : 1 - لا يوجد فرق ذو دلالة احصائية عند مستوى (05.0) بين مت | 50 isolates of the bacteria Pseudomonas aeruginosa was obtained from 201 clinical samples were distributed between Burn Specialist Hospital and teaching laboratories from patients with burns, infected wounds, middle ear infection, urinary tract infection and respiratory tract infection for a period from 1st February till 1st May. And we obtained 50 bacterial isolates from 20 samples from contaminated and non - contaminated soils were distributed different areas of Baghdad for a period of 1st May 2013 till 1st August.Number of cultural microscopically, biochemical and sensitivity to antibiotics tests had been, than diagnosis was confirmed by API20E system.These isolates was tested for ability to production of biosurfactants (rhamnolipid) by haemolysis, oil spreasding test, calculate value of emulisification factor (E24) and measuring surface tension for liquid media. Tow isolates (PS42 and PP8) had been selected, first one was from soil samples and other was from pathological samples because of they have highest productivity, haemolysis ability, oil spreading, highest emulisification factor value and highest in lowering surface tension, there for these tow isolates selected for study their inhibitory activity against types of bacteria. The rhamnolipid was extracted from tow isolates P. aeruginosa PP8 and P. aeruginosa PS42 by using mixture of solvents as was obtained 15.45 g and 18.25 g per liter of each of the PP8 and PS42 respectively. The rhamnolipid was diagnosed by thin layer chromatography technology (TLC) and high - performance liquid chromatography (HPLC) showed that tow bacteria produced three types of rhamnolipids (mono - , di - rhamnolipid and rhamnolipid A). The rhamonlipid efficiency was tested for inhibitory activity against bacteria by measuring diameter of inhibitory zone surrounding holes and discs. The inhibitory activity was high against Bacillus cersus bacteria followed by P. aeruginosa, than Staphylococcus aeraus and the lowest one was E. colli. The inhibitory activity for biosurfactant was approached to inhibitory activity for industrial surfactants. Also in this study has been determined minimum inhibitory concentration (MIC) value and Minimum Bactericidal Concentration (MBC) value for biosurfactants and the results varied depending on different types of biosurfactants and type of bacteria and the lowest values for MIC and MBC of biosurfactant produced by P. aeruginosa PS42 on growth of B. cereus and reached to 16.It was first time at local level for testing inhibitory activity of biosurfactant against pathogenic bacteria S. aureus and P. aeruginosa which was studied in vivo (injuried skin) after introduction these pathogens experimentally into laboratory mice with clinical symptoms appeared in injured skin after 48 hours and then treated mice groups with of 30 mg \ ml concentration of biosurfactants produced from isolates of bacterial (P. aeruginosa PP8 and P. aeruginosa PS42), resulted in a decrease in the time required for healing as found from the results obtained by the different length of period needed for healing (depending on the nature of injury and type of microorganism that causes injury).The mice had been recovered after 5 days when used biosurfactant at concentration 30 mg/ml produced P. aeruginosa PS42 followed by mice recovered after 6 days when used biosurfactants 30 mg/ml produced by P. aeruginosa PP8 in comparison with control group which recovered after 12 days for mice infected with S. aureus. In mice infected with P. aeruginosa recovered after 10 days when used biosurfactant produced by P. aeruginosa PS42 at concentration 30mg/ml and followed by mice recovered after 12 days when used biosurfactant produced by P. aeruginosa PP8 at concentration 30mg/ml in comparison with control group which recovered after 17 days.

Abstract: تضمنت الدراسة الحالية اجراء مقارنة حول نسب انتشار الطفيليات المعوية وعلاقة الاصابة ببعض مكونات الدم المحيطي (اقيام الهيموغلوبين والعدد الكلي لكريات الدم البيض واعداد الحمضات) في مرضى من منطقتي ابو غريب والعامرية خلال المدة مابين شهر تشرين الاول 2011 ولغاي | The present study has included the comparison 0f prevalence rates of intestinal parasites and the relationship of infection of certain components peripheral blood (hemoglobin values, the total number of white blood cells, eosinophils numbers) in patients of each of the regions of Abu Ghraib and Amiriyah during the period from October, 2011 until July, 2012 as it has been collected (2449) stool samples that were distributed between (1430) stool samples from Abu Ghraib Hospital auditors and (1019) stool samples of auditors of the Health Center in Amiriya and also it has been collected blood samples. Stool samples were examined by direct method using brine (Normal Saline) and tincture of iodine (Lugol's Iodine) and the floating way by sulphate zinc water (ZnSo4.7H2O) to investigate the prevalence of intestinal parasites and the relationship of infection by age, sex, number of family members, and the type of water used for drinking. The results showed the presence of significant difference at the possibility(p<0.05) in total infection for the regions of Abu Ghraib and Ameriyah where the percentage was (36.29%) in the area of Abu Ghraib and (27.57%) for the Amiriyah area. It has been diagnosed five types of intestinal parasites (protozoa + Helminthes) as follows : A - Intestinal protozoa in the Abu Ghraib and for the Amiriyah area : 1 - Entamoeba histolytica was (17.62%, 16.68%).2 - Giardia lamblia was (11.95%, 5.49%).3 - Entamoeba coli colon was (2.44%, 2.55%(.B - Helminthes1 - Dwarf tapeworm Hymenolepis nana was (2.37%, 1.76.(2 - pinworm Enterobius vermicularis was (1.88%, 1.07.(We Did not observe any significant differences between the infection of intestinal parasites and sex factor where the total infection for males was (6.53%) while for females, it reached (8.06%) for Abu Ghraib, as to the Amiriya region, also we did not notice any significant differences between the infection of intestinal parasites and sex factor where The total infection for males was (7.26%) while for females, it was (6.84%(. It has been also found that there are significant differences and when probability of (P <0.05) for the infection with parasite (Giardia lamblia) between the areas of Abu Ghraib and Amiriyah where infection rate reached (11.9%) in Abu Ghraib while for the Amiriyah area, it has reached the ratio of, (5.49%) for the same parasite, while there was no significant difference between the other four types of parasites. The individual injuries are prevalent Lama has reached bilateral injuries (56) bilateral injury in the Abu Ghraib area, and (13) in the area of bilateral injury Ameria and most of the injuries were bilateral parasite (E.histolytica+ G.lamblia). The results showed that there is a significant difference when probability (P <0.01) for the distribution of age groups, reaching the highest infection rate in the age group of (29 - 20) years, (56.68%) and the lowest infection in the age group (60 years and over) the percentage of infection was (21.42%) in the Abu Ghraib area, while for Amiriyah area, it has been recorded the highest infection rate in the age group of (29.20), where it reached (49.22%) and the lowest injury in the age group (60 years and above) where The percentage of infection was (4.82%(. Also, it was noticed the emergence of variation in the values of the components of the peripheral blood and the lack of significant difference when probability (P <0.05) in the area of Abu Ghraib where it recorded the highest proportion of hemoglobin in the parasite (Entamoeba histolytica), amounting to (12.40 g / 100 ml) and the lowest percentage hemoglobin was recorded in parasite (Enterobius vermicularis) amounting to (9.46 cells / mm3), and the lack of a significant difference in the total number of white blood cells where it recorded the highest rate in the parasite (Entamoeba histolytica) 6776.32 cells / mm3 while the lowest rate was recorded in the parasite (Enterobius vermicularis (5074.37cell\mm3, and lack of a significant difference for eosinophile numbers was recorded the highest proportion in parasite Hymenolepis nana 516.63 cell\mm3 and the lowst rate was recorded in parasite Entamoeba histolytica 159.63 cell\mm3. As for the Amiriyah, it hasn’t been noticed any significant differences when probability (P <0.05), where the highest proportion of hemoglobin in the parasite (Entamoeba histolytica), was (11.76 g / 100 ml) and the lowest percentage hemoglobin recorded in parasite (Enterobius vermicularis) was (9.90 g / 100ml), and there was the lack of a significant difference in the total number of white blood cells, where it has been recorded the highest rate in the parasite (Entamoeba histolytica) 6574.59 cells / mm3 and the lowest rate was recorded in the parasite (Entamoeba coli) 5700.00 cells / mm3 and the lack of a significant difference for eosinophile numbers. The highest proportion in parasite Enterobius vermicularis was 518.65 cell\mm3 and the lowst rate in parasite Entamoeba coli was 140.29 cell\mm3. The results also showed that there is a high significant difference when the probability (P <0.01) in the incidence of intestinal parasites and the relationship of water used for drinking where the study proved that the highest infection rates recorded of the people who use canal waters and water tank where It percentages was (49.54%, 39.00%), respectively while the lowest rate of infection was recorded in people who use boiling water which reached (14.02%). The same applies to the Amiriya district which recorded the highest percentage of people who use tap water and the lowest percentage of those using boiling water reaching ratios (41.49 %, 12.5%), respectively.

Abstract: بحثت الدراسة جمع 100 مسحة من اطفال مصابين بالتهاب اللوزتين الحاد والمزمن ممن راجعوا استشارية الانف والاذن والحنجرة في مستشفى اليرموك التعليمي للمدة مابين كانون الاول للعام 2013 ولغاية اذار للعام 2014. وكانت نتيجة الزرع المختبري موجبة في 67 مسحة منها وبالا | In this study 100 swabs were collected from infected children with acute and chronic tonsillitis who attended at Al - Yarmook Teaching Hospital (Eear Nose Tonsils consultation clinic) from December 2013 until March 2014. The result of laboratory culture were positive in 67 samples. Depending on their cultural, morphological and biochemical characterization of bacterial isolates : (37.31%) of them were identified as Streptococcus pyogenes bacteria and the diagnosis is confirmed by the use of Remel Rapid STR System. Whereas (34.33%) belong to S.parasanguinis, (11.94%) S.mitis, (11.94%) S.oralis and (4.48%) S.thoraltensis were identification of them are confirmed by using Vitek - 2 System also. As well as Sensitivity test for S.pyogenes cultures for some of the antibiotics done and the results revealed that all cultures were sensitive to tetracycline and chloramphenicol drugs 100%, while it was different in their sensitivity for other drugs. According to age (2 - 13) years revealed the highest percentage of infection for both male and female, however the percentage of acute and chronic tonsillitis was higher in males than females. In this study, during investigation of the antagonist action of lactic acid bacterial isolates against these pathogenic bacteria, results revealed that the method of L.acidophilus discs gave a high inhibition zones with a diameter of 11.3 mm, while L.fermentum discs yielded a high inhibition zones with a diameter of 7 mm 24 hrs following incubation in comparison with well diffusion method by using suspension of L.acidophilus produced a high inhibition zones with a diameter 7.25 mm 48 hrs after incubation. L.fermentum suspension produced a high inhibition zones with 10 mm diameter 24 hrs following incubation. Also the study included measurement of the inhibition activity for bacteriocins produced by L.acidophilus bacteria against the pathogenic bacteria S.pyogenes on nutrient agar by well diffusion method in which results revealed stability of the bacteriocins effects under different PH (4, 5, 6) with percent (100, 60, 50)% respectively for 24 hrs at higher stability and the highest stability of bacteriocins during PH was 4 with percent 100%, while it lost a lot of its activity with acidic PH less than 2 and basal pH more than 8. During the study the effect of Nacl, Kcl and MgSo4 with different concentrations (1 - 5%) with constant inhibitory effect for bacteriocins produced by L.acidophilus against pathogenic S.pyogenes, The result revealed that Nacl had little effect in inhibition zone with 1&2% concentrations. The salt MgSo4 and Kcl showed reduction in the inhibitory activity with 1, 2, 3% concentration, however the higher concentration of salt caused great reduction as 5% concentration led to loss of inhibitory activity for bacteriocins completely.Also this study showed light on the inhibitory effect of probiotics suspensions against the adhesion property of S.pyogenes after treated with L.acidophilus and L.fermentum.where the Result shown that L.acidophilus inhibit the adhesion of S.pyogenes with percent 68.92% either L.fermentum inhibit the adhesion of this pathogenic bacteria to epithelial cell with percent 30.28%. The study is consisted of showed the inhibitory action for suspensions of L.acidophilus and L.fermentum on biofilm formation by S.pyogenes, it was noticed the action of L.acidophilus in the side more than from the action of L.fermentum. During the study of the effect of suspension and filter of L.acidophilus and L.fermentum on the production of Hemolysin enzyme by S.pyogenes, it was noticed that the ability of suspension belong to L.acidophilus and L.fermentum to reduce the diameter of hemolysis zone which was beta type more than the ability of filter L.acidophilus and L.fermentum to reduce of the hemolysis zone which was beta type.

Abstract: لحصول على عزلات Staph. epidermidisتم جمع (160 عينة) من حالات سريرية مختلفة كالتهاب المجاري البولية, التهابات جلدية, البلعوم, الانف, والاذن. تم الحصول على 51 (875¸31?) عزلة للمكورات العنقودية الجلدية Staph. epidermidis خلال فترة امتدت من ايلول 2014 الى كان | In order to isolate Staph. epidermidis, 160 clinical specimens were collected from (urinary tract infection, skin infection, pharynx, nose, and ear). Only 51(31.875 %) Staph. epidermidis isolates were obtained during a period from September 2014 to January 2015, depending on some biochemical tests and VITEK2 system. The Staph. epidermidis was given (gram stain, catalase , urase) positive, (coagulase, manital fermentation, oxidase, motility) negative, non haemolytic to human blood with some exception, most of the strain were able to produce biofilm, and (100%), (64.70%), (74.50%) resistant to penicillin G, ampicillin, and amoxicillin respectively. The Staphylococcus species identified during study were 44 (27.5%) isolates of Staph. aureus, 5 (3.125 %) Staph. haemolyticus, 4 (2.5 %) Staph. saprophyticus, 8 (4%) isolates other Staphylococcus spp. such as Staph. hominis ssp. hominis, Staph. capatis, Staph. xylose, Staph. simulanus, Staph. lentus, and 48 (30 %) from unknown genera. To isolate bacterioghages from sewage water, several sewage water samples were assayed using plaque assay of double agar overlay as a source of Staph. epidermidis phages. The bacteriophages were described depending on plaques size and shapes, phage 1 was the most predominant and frequent in the bacterial lawn and able to infect other S. species such as S. aureus. It has been selected to study it is titer, latent, rise period, and burst size were calculated. The effect of temperature, pH , and NaCl ions on it is original titer were studied.The results revealed a gradual decreasing in the phage titer with increasing dilution number. Latent period extended to (30 minutes) , while rise period was started with (40 minutes) extending to (60 minutes) , burst size was 2.346. Each temperature at several incubation periods , pH , and NaCl ions was significantly varied depending on phage titer. The optimum temperature was 40 ° C, while the 80 ° C was represented the inhibitor temperature. L.S.D. at level (0.05) for interaction was 39.552. The pH 6.5 - 7.5 represented the optimal pH for the best phage activity while the phage titer beginning to declining in above and below this range of optimal pH, L.S.D. at level 0.05 was 17.898 , the optimum NaCl ions concentrations were (0.1 M and 0.25 M), while the titer was significantly decreased with increasing the NaCl ion concentration in the culture solution, the L.S.D. at level 0.05 was (10.696). In conclusion of this study found that Phage1 was considered as predominant phage because of their ability to infect other Staphylococci species such as Staph. aureus.
