Abstract: Fifty clinical samples suspected as Pseudomonas aeruginosa were collected from patients with different diseases, diabetic foot ulcers, U.T.I., wounds, burns, keratitis, external otitis from September 2015, till February 2016, from hospitalized patients in four different Baghdad hospitals (Al - Kindi General Teaching Hospital, Al - Yarmuk General Teaching Hospital, Alkadhmiya General Teaching Hospital, and AL - Shiakh Zaied Hospital). The study population targeted both gender from different ages groups. The collected samples were cultured on different selective and differential media and tested biochemically in order to identify P.aeruginosa, then the identification was ensured by using two molecular methods which are conventional PCR in the detection of ecfx gene and 16SrDNA gene and real time PCR with commercial kit from sacace (qualitative detection of P.aeruginosa) and with gyrB gene using specific primers to each gene.Sensitivity for each test was measured then compared with other tests to determine the test efficiency. The results showed that the sensitivity for API20NE test was (80%), the sensitivity for molecular tests as flows : ecfx test was (92%), 16SrDNA test was (96%), the commercial kit test was (98%) and for gyrB test was (92%).The present study shows that using more than one target gene is reducing the probability of misidentification of P.aeruginosa that may be caused by variation in primer or probe targets.According to the type of infection the comparison was done. The results of P.aeruginosa detection for diabetic foot isolates were compared with other isolates. The results showed that there is a remarkabledifference in the API20NE test; as well as, with the oxidase test (according to the time that oxidase test need it to give the dark purple color as a positive result), for the mucoid strains of this bacterium. The results of detection isolates by using gyrB gene were differed too. Then a comparison in simplicity, cost, accuracy and time of P.aeruginosa identification between molecular methods and API20NE test (biochemical test) and between molecular tests themselves were performed. The results of these comparisons show that the molecular methods are better than API test in all aspects of the comparison. Real time PCR is better than conventional PCR in simplicity, accuracy and time but not in cost. All isolates were tested for antibiotic sensitivity using nine antibiotics. The results showed that most isolates were sensitive to Levofloxacin (94%), Ticarcillin - clavulanic acid (Timinim) (88%), Gentamicin (82%) and (80%) to both Pipracillin and Imipenem, while the isolates appeared resistant to another antimicrobials represented by (72%) to Augmentin, (34%) to Ceftazidime, (38%) to Colistin, and (28%) to Ciprofloxacin. In the present study, Levofloxacin represents the most effective antibiotic against P. aeruginosa isolates with the all types of infection. The present study improves the importance of the rapid and accurate detection of the bacterial types and thus giving the right antibiotics to patients to obtain rapid recovery and prevent any complications that may lead to death.

Abstract: Urine and blood samples was Collected 245 from patients who were resident of the industrial renal unit of Al - kadhamyiah (p) Teaching Hospital and Al - kindy Teaching Hospital from kidney failure seeking medical care for all ages and both sexes for a period between 05/11/2014 - 13/1/2015 .The samples were cultured in a suitable culture media for isolation bacteria. Diagnosed bacteria isolation on agar medium accuracy and privacy (Chromagar Orientation medium) as dependent as a result of the diagnosis on the color interaction between the enzymes of different bacterial isolates and the components of the center, specialist this confirmed diagnosis using API 20E and API Staph system results showed that the percentages of the bacterial isolates were as follows The results showed that 140 samples were positive for bacterial growth, It included 66 urine samples, has been negative bacterial 61/66 (92.42%) the isolation of prevailing compared bacterial positive grams for gram - stain and number 5/66 isolates, which accounted for (7.58%) of urine, while the number of bacterial isolates negative for tinted grams 26/74 (43.51%) compared to the isolation positive for gram - stain and number of 48/74 (64.86%) bacterial isolates . results show that the bacteria E .coli was the most sought after the generation of patients with kidney failure as it was isolated 57.57 percent, and by 18.92% isolated from the blood of patients with kidney failure. K.pneumoniae bacteria, followed by 25.75%, then the bacteria S.aureus by 7.57% and were S. aureus bacteria are prevalent in the blood as isolated 45.59%, then all of P. mirabilis and P. aeruginosa bacteria 4.54% P.aeruginosa isolates ratio was one of the blood 13.51%. Isolated the bacteria E.cloacae and Enterococcus spp 2.70% - 18.92% of the blood samples .The results showed that the age groups most affected renal failure were in the age group (50 - 70 years) as showing that the disease affects males and females, the study showed that there is an increase significantly in some measures of patients, male and female , and these standards is protein acute phase, the low increase significantly in the level of IGg and molecules complement C3,C4 in the serum of patients. results showed significant decrease in response to IL - 4 in patients level average of 297.4 pg \ ml compared with the levels in the control group, which recorded 410.1 rate pg \ ml and was a significant decrease in the level P< 0.05. Results show the high level of IL - 17A ,but not significant in patients with Group 79.2 pg \ ml compared with the levels in the control group, average of 76.6 pg \ ml.Results showed a high level of IP - 10 concentration in patients, amounting to an average of 0.318 pg \ ml compared with a total of 0.282 pg control \ ml at the level P< 0.05. results recorded a high level of response for MCP - 1 in patients, amounting to an average of 186.1 pg \ ml compared with normal levels in 116.2 when the total control pg \ ml moral worth at the level P< 0.05. The results of the study slight rises in cytokine rate significant difference was not a statistically significant in all cytokine between males and females have the current study signals are consistent with another study that sex is not at the level of cytokine in the serum of patient.
