Abstract: في اول دراسة في العراق ، تم قياس مستوى الكانكليوسيدات لغرض تقييم دوره لدى الاطفال الخدج . لقد تم دراسة 300 طفل حديثي الولادة وقد تم تقسيمهم الى 4 مجاميع وتضم ، 100 حديثي الولادة يبدون اصحاء ذوي اوزان طبيعية ، 100 خديج يبدون اصحاء ذوي اوزان قليلة ، 50 خديج مصابين بالاختناق الولادي ذوي اوزان قليلة ، 50 خديج مصابين بمتلازمة عسر التنفس ذوي اوزان قليلة . وقد تم مقارنة النتائج المستخلصة بينهم . لقد تم قياس معايير حياتية في مصل الدم لحديثي الولادة ومجاميع المقارنة ، وان هذه المعايير تشمل : الكانكليوسايدس ، حامض السياليك الكلي ( TSA) ، حامض السياليك البروتيني ( PASA) ، مالوندايالدهايد ( MDA) ، كلوكوز ، كولسترول ، ترايكليسرايد ، هيموكلوبين ( Hb) ، البليروبين الكلي في مصل الدم ( TSB ) ، الالبومين ، والبروتين التفاعلي - س . وقد اظهرت النتائج ان : - 1. ان معدل مستويات الكانكليوسيدات في مصل الدم لدى حديثي الولادة ذوي الاوزان القليلة هو ( 0ر14 ملغم / ديسـي لتر ) كان اقل بصورة دالة عن ما هو عليه في حديثي الولادة ذوي الاوزان الطبيعية ( 94ر16 ملغم / ديسـي لتر ) ، ولكن لدى ذوي الاوزان القليلة المصابين بالاختناق الولادي كان ( 26ر25 ملغم / ديسـي لتر ) وذوي الاوزان القليلة المصابين بمتلازمة عسر التنفس ( 92ر24 ملغم / ديسـي لتر) اذ كان اعلى بصورة دالة عن المعدلات في مصل الدم لدى حديثي الولادة ذوي الاوزان القليلة وذوي الاوزان الطبيعية . 2. ان معدل مستويات حامض السياليك الكلي لدى حديثي الولادة ذوي الاوزان القليلة ( 54 ر43 ملغم / ديسـي لتر ) كان اقل بصورة دالة عن ما هو عليه لدى حديثي الولادة ذوي الاوزان الطبيعية ( 46ر51 ملغم / ديسـي لتر ) . 3. ان معدل مستويات حامض السياليك البروتيني في مصل الدم لدى حديثي الولادة ذوي الاوزان القليلة ( 03 ر28 ملغم / ديسـي لتر ) اذ كان اقل بصورة دالة عما هوعليه لدى حديثي الولادة ذوي الاوزان الطبيعية ( 53 ر33 ملغم / ديسـي لتر ) . 4. ان معدل مستويات المالوندايالدهايد في مصل الدم لدى حديثي الولادة ذوي الاوزان القليلة ( 63ر0 نانو مول / مللي لتر ) وكان اعلى بصورة دالة عما هو عليه لدى حديثي الولادة ذوي الاوزان الطبيعية ( 49 ر0 نانو مول / مللي لتر ) . 5. ان معدل مستويات الكلوكوزفي مصل الدم لدى حديثي الولادة ذوي الاوزان القليلة ( 56 ر2 مللي مول / لتر ) ليست مختلفة بصورة دالة عما هو عليه لدى حديثي الولادة ذوي الاوزان الطبيعية ( 21ر2 مللي مول / لتر ) .6. ان معدل مستويات الكلسترول في مصل الدم لدى حديثي الولادة ذوي الاوان القليلة ( 79ر1 مللي مول / لتر ) كان اقل بصورة دالة عما هو عليه لدى حديثي الولادة ذوي الاوزان الطبيعية ( 21ر2 مللي مول / لتر ) . 7. ان معدل مستويات التراكليسرايد في مصل الدم لدى حديثي الولادة ذوي الاوزان القليلة ( 53 ر0 غم / لتر ) كان اقل بصورة دالة عما هو عليه لدى حديثي الولادة ذوي الاوزان الطبيعية ( 63ر0 غم / لتر ) . 8. ان معدل مستويات الهيموكلوبين في الدم لدى حديثي الولادة ذوي الاوزان القليلة ( 63ر15 غم / ديسـي لتر ) ليست مختلفة بصورة دالة عما هو عليه لدى حديثي الولادة ذوي الاوزان الطبيعية ( 28ر15 غم / ديسـي لتر ) . 9. ان معدل مستويات البليروبين الكلي في مصل الدم لدى حديثي الولادة لدى الاوزان القليلة ( 9ر22 مايكرومول / لتر ) ليست مختلفة بصورة دالة عما هو عليه لدى حديثي الولادة ذوي الاوزان الطبيعية ( 7ر24 مايكرومول / لتر ) .10. ان معدل مســتويات الالبومين في مصل الدم لدى حديثي الولادة ذوي الاوزان القليلة ( 88 ر36 غم / لتر ) كان اقل بصورة دالة عما هو عليه لدى حديثي الولادة ذوي الاوزان الطبيعية ( 81 ر39 غم / لتر ) . 11. كانت النتائج الايجابية في مصل الدم للبروتين التفاعلي - س لدى ذوي الاوزان القليلة المصابين بالاختناق الولادي ولدى ذوي الاوزان القليلة المصابين بمتلازمة عسر التنفس ( 20 % و18 % على التعاقب ) اكثر مما وجد لدى حديثي الولادة ذوي الاوزان القليلة وذوي الاوزان الطبيعية ( 14% و9% على التعاقب ). الاستنتاج : ان معدل مستويات الكانكليوسيدات قد وجدت اكثر بصورة دالة لدى المرضى ذوي الاوزان القليلة بالمقارنة مع ذوي الاوزان القليلة الاصحاء ظاهريا" وذوي الاوزان الطبيعية الاصحاء ظاهريا". | In a pioneer study conducted in Iraq, the serum levels of gangliosides were tested to evaluate their role in premature neonates. The study had been done on 300 neonates; they were divided into 4 groups, and these includes 100 apparently healthy normal birth weight mature neonates (NBW), 100 apparently healthy low birth weight premature neonates (LBW), 50 low birth weight premature neonates with birth asphyxia (LBW+BA), and 50 low birth weight premature neonates with respiratory distress syndrome (LBW+RDS). The following biochemical parameters were measured in sera of the studied neonates and the control groups, and these parameters included : gangliosides, total sialic acid (TSA), protein associated sialic acid (PASA), malondialdehyde (MDA), glucose, cholesterol, triglyceride, total serum bilirubin (TSB), albumin, C - reactive protein, and blood hemoglobin (Hb). The results revealed that : 1. The mean serum gangliosides level in LBW neonates (14.0 mg/dl) was significantly lower than that in NBW neonates (16.94 mg/dl), while in LBW+BA (25.26 mg/dl) and LBW+RDS (24.92 mg/dl) were significantly higher than the mean serum levels in LBW and NBW neonates. 2. The mean serum TSA level in LBW neonates (43.54 mg/dl) was significantly lower than that in NBW neonates (51.46 mg/dl).3. The mean serum PASA level in LBW neonates (28.03 mg/dl) was significantly lower than that in NBW neonates (33.53 mg/dl). V4. The mean serum MDA level in LBW (0.63 nmol/ml) was significantly higher than that in NBW neonates (0.49 nmol/ml).5. The mean serum glucose level in LBW (2.56 mmol/L) did not significantly differ from that in NBW neonates (2.69 mmol/L).6. The mean serum cholesterol level in LBW (1.79 mmol/L) was significantly lower than that in NBW neonates (2.21 mmol/L).7. The mean serum triglyceride level in LBW (0.53 g/L) was significantly lower than that in NBW neonates (0.63 g/L). 8. The mean blood Hb level in LBW (15.63 g/dl) was not significantly different from that in NBW neonates (15.28 g/dl).9. The mean TSB level in LBW (22.9 µmol/L) was not significantly different from that in NBW neonates (24.7 µmol/L).10. The mean serum albumin level in LBW (36.88 g/L) was significantly lower than that in NBW neonates (39.81 g/L ).11. The positive results of serum C - reactive protein in LBW+BA and LBW+RDS (20% and 18% respectively) were more than that found in LBW and NBW neonates (14% and 9% respectively). In conclusion; the mean serum gangliosides level was found to be significantly higher in diseased LBW in comparison with the apparently healthy LBW and the apparently healthy NBW neonates.
