Abstract: هدفت الدراسة الحالية الى التحري عن الانماط الوراثية وعوامل الضراوة في البكتريا القولونية الممرضة ومقارنتها مع البكتريا القولونية المتعايشة وعلاقتها بجين مقاومة مضادات البيتالاكتام blaCTX-M. تم الحصول على 125 عزلة من الاشريكية القولونية ) 100 ممرضة و25 متعايشة( من اصل 380 عينة من اصابات المجاري البولية والجروح والحروق و25 عزلة من براز Stool الاشخاص الاصحاء لكلا الجنسين ولمختلف الاعمار من مستشفيات مختلفة في محافظة النجف الاشرف وللفترة من السابع والعشرون من تموز 2022 ولغاية الحادي عشر من كانون الثاني 2023 وبعد تشخيص العينات مختبريا عن طريق زراعتها على الاوساط الانتقائية )الماكونكي والايوسين مثيلين الازرق( اضافة الى ذلك للتاكيد التشخيص باستخدام جهاز الفايتك VITEK-2 compact system توزعت العزلات الجرثومية الممرضة على 71 عزلة من الاناث و20 عزلة من الذكور المصابين بااصابات المجاري البولية ، اما الجروح بلغت 7 عزلات ، والحروق فقد بلغت 2 من الذكور، وبلغت نسبة الاصابة في كل من الذكور حوالي 25% اما الاناث بلغت 75% ، اما بالنسبة الى العزلات البرازية جمعت 25 عزلة متعايشة تضمنت الاناث 4 ( 16% ( اما الذكور بلغت حوالي 21 84%) ( ، وتم اجراء اختبار الحساسية لبكتريا الاشريكية القولونية لعدد من المضادات الحياتية لكل من العزلات الممرضة والمتعايشة . بينت نتائج الدراسة الحالية نسبة المقاومة للعزلات الممرضة اذ بلغت نسبة المقاومة لمضاد Piperacillin كانت 64%) 64 ( ومضاد Amoxicillin – Clavulanate بلغت 63 (63%) اما لمضاد Ceftriaxone كانت نسبة المقاومة 43 (43%) ولمضاد Ciprofloxacin بلغت نسبة المقاومة 34 (34%) ولمضاد Ceftazidime بلغت نسبة المقاومة 33 33%) ( ولمضاد Trimethoprim- Sulfamethoxazole كانت المقاومة 29 29%) ( وبلغت نسبة المقاومة لمضاد Levofloxacin (25%) 25 ولمضاد Doxycycline كانت نسبة المقاومة حوالي (20%) 20 ولمضاد Aztreonam بلغت حوالي 19 (19%) ولمضاد Cefoxitin كانت 15 (15%) ثم المضاد Gentamicin كانت نسبة المقاومة حوالي (5%) 5 واظهرت مقاومة ضعيفة جدا لمضاد Imipenem وAmikacin اذ بلغت حوالي 1 (1%) اما بالنسبة الى العزلات المتعايشة بلغت نسبة المقاومة Amoxicillin – Clavulanate حوالي (88%) 22 ولمضاد Piperacillin حوالي 17 (68%) ولمضاد Ceftriaxone (64%)16 ولمضاد Ceftazidime (56%)14 ولمضاد Cefoxitin كانت (24%)6 ولمضاد Aztreonam بلغت (24%)6 واخيرا لمضاد Imipenem كانت نسبة المقاومة 0%)0 . وتم الكشف عن المجاميع الوراثية للعزلات الممرضة والمتعايشة باستخدام تقنية تفاعل انزيم البلمرة المتسلسل Multiplex اذ اظهرت النسب المجاميع الوراثية للعزلات الممرضة اذ وجدت اعلى نسبة تعود للمجموعة B1 حيث بلغت (44%) 44 وتليها المجموعة B2 حيث كانت 43 43%) ) ومن ثم D 12 ( 12% ( واخيرا A 1 ( 1% (. اما العزلات المتعايشة كانت اعلى نسبة تعود الى المجموعة B1 اذ بلغت حوالي (72%)18 تليها B2 6 ( 24% ( ثم المجموعة D 1 ( 4% ( ولا توجد نسبة تمثل المجموعة A . تم التحري عن جينات الضراوة لكل من العزلات الممرضة والمتعايشة ، حيث بلغت نسبة الجين fimH حوالي ) 32 62.7% (، وللجين aer 45.0%)23 (، وhlyA (11.7%)6 ، اما بالنسبة الى العزلات المتعايشة بلغت نسبة الجين fimH حوالي 40%)6 ،) (86.6%)12 aer ، hlyA 2 ( 13.3% . ومن ثم تم ربط المجاميع الوراثية بجينات الضراوة ولكل من العزلات الممرضة والمتعايشة اذ ظهرت في العزلات الممرضة ، ارتباط الجين fimH بالمجموعة B2 بنسبة 29.4%)15 (، وتليها المجموعة B1 19.6%)10 (، وD 11.7%)6 (، واخير A 1 ( 1.9% (. اما الجين aer اعلى نسبة تعود الى المجموعة B2 27.4%)14 (، وتليها B1 13.7%)7 ،) D بلغت 3.9%)2 (. اما الجين hlyA B2 6 ( 11.7% (. اما بالنسبة الى العزلات المتعايشة للجين fimH وجدت اعلى نسبة للمجموعة B2 26.6%)4 (، وتليها B1 13.3%)2 (، اما للجين aer B2 40%)6 ،) B1 33%)5 ،) D 1 ( 6.6% (، وللجين hlyA B2 13.3%)2 . وتم التحري عن جين blaCTX-M في كلا العزلات اذ وجد نسبة الجين في العزلات الممرضة 57.5%)23 (، وللعزلات المتعايشة بلغت (12.5%)2 وجد ان هناك علاقة ما بين المجاميع الوراثية وجينات الضراوة وانتاج CTX-M اذ كانت المجموعتين الوراثية B2 وB1 هي الاعلى من حيث احتوائها على انواع مختلفة من جينات الضراوة وانتاج انزيم CTX-M تلتها D اما بالنسبة الى العزلات المتعايشة اذ كانت المجموعة B2 هي الاكثر احتواء على جينات الضراوة وكانت فقط عزلة واحدة منتجة CTX-M تلتها B1 احتوت على جينات fimH, aer وعزلة واحدة انتجت CTX-M ومن ثم المجموعة D ولم تنتج CTX-M

Abstract: جمعت (113) عينه سريريه (مسحة اذن Ear swab) لمرضى التهاب قناة الاذن الخارجيه الفطري Otomycosis من كلا الجنسين والذين راجعوا استشارية الانف والاذن والحنجره في مستشفى الصدر التعليمي والعيادات الخاصه للاطباء الاختصاص / محافظة النجف الاشرف للمده من تشرين الثان | This study included 113 ear swabs from patients with otomycosis, males and females, visited the otolaryngology consultation room at Al - Sader Medical City in An - Najaf / Iraq in addition to the private otolaryngology clinics of some of ear, nose and throat specialists, during the period from October 2012 to March 2013, the study aimed to isolate and diagnose the causative opportunistic mycotic infections of the ear and to test sensitivity of these organisms towards antimycotic agents in addition to identifying some enzymes of the most fungal reluctant type phenotypically and molecularly. (103) positive swabs were obtained from total samples including "Cephalosporium, Candida spp, Aspergillus spp, Neurospora and Trichosporium beigelii.The Precentage of isolated fungi was (85.43)% for Aspergillus, (10.67)% for Candida, while other species constituted totally (3.88)%. The study show that the highest percentage of isolated fungi was from males as it was(51.4)% while in females was (48.6)%. Patients' ages were found to be between (11 - 80) years and the age group (21 - 30) years was the most frequent Category in (30.3)% and the Category (71 - 80) years was affected only in (0.8)%. The highest percentage of insulation fungi during the period of sample collection was in February which constitute (30.9)% and the lowest percentage was in March reaching only (14.1)%.The highest ratios to isolate the fungus was recorded among housewives and unemployed males and it was (44.2%), while the lowest rates of insulation when the students reached (15.9%).The cases of otitis media excelled on the rest of the pathological cases accompaniments of Otomycosis as the proportion of insulation was (19.4%), followed by disease of blood pressure which their percentage was (5.3%), while the rest of the pathological cases did not have significant influence in insulation, for more ear side affected the left ear was affected in(51.8)% and the right was affected in (48.2) %. The study showed that there is inhibitory effect of (flucanazole, Nystatin, Clotrimazole, Miconazole, Itraconazole and Ketoconozole) against Aspergillus spp (A.niger, A.flavus and A.terreus). All antifungals were effective and their inhibitory ratio was (100) %, except for the antifungal (Fluconazole ) which found to have a lower inhibitory effect (10) % against the species A.flavus only. Testing the phospholipase activity for aspergillus species, A.flavus and A.niger showed phospholipase activity in two degrees (++, +++), but A.terreus shoed only (++). Searching the gene alp " which is encoded for the production of alkaline protease " showed that all the Aspegillus species produces the gene with amplification results of (100)% for the band supposed to appear at the molecular weight of (747) pb
