Abstract: Sickle cell anemia (SCA) is a known hematological disorder that arises from a single point mutation in codon 6 of the β-globin gene that results in a glutamic acid to valine substitution. This mutation leads to the formation of abnormal hemoglobin called HbS. Homozygous hemoglobin S (HbSS disease) is the most common form of SCD, In addition to homozygous SCD (HbSS), other forms such as HbSC and HbSB thalassemia also exist. This study has been conduct during 3 months between November/2022 and January /2023 in the hereditary blood disease center / Karbala Health Directorate. The total number of participants are (100) person including (35) patients with painful crisis, (35) patients with steady state, and (30) control group, all of the patients and control group were male, in addition, There ages ranged from (3-55). Patients and healthy individuals provided venous blood samples totaling 5 ml. Each person also provided personal information, including their age, height, and weight. visfatin, and vcam-1 were done by manual kit with Enzyme-linked ImmunoSorbent Assay , while ferritin, Complete Blood Count, C-Reactive Protein, D-dimer, and Lactate dehydrogenase were done by auto chemistry analyzer. In this study, we aimed to study the role of VCAM-1 as a marker of endothelial dysfunction in SCA patients. Investigation of the role of visfatin, as a marker of inflammation in SCA patients. Compared the expression of visfatin, sVCAM-1, Ferritin, and D dimer, in SCA patients that to control group. Comparing the severity of these markers D dimer, Ferritin, visfatin, and sVCAM-1 in children and adults. Evaluate the association between serum visfatin, sVCAM-1 level in SCA patients, and the frequency of vaso-occlusive crises. Possible usage of these markers as a X predictive index for VOC occurrence. Study correlations among visfatin, sVCAM-1, Ferritin, and D dimer markers with different hemolysis and inflammation markers. The results showed there has been significant differences in concentrations of D dimer, Ferritin, visfatin, CRP, HCT, sVCAM-1, LDH, HB, WBC, RBC, and Retic count, in SCA patient (painful crisis, steady state) in comparison with the control group for both adults and children. Also observed no significant differences in MCH levels between SCA patients and the control group for both adults and children. The results of MCHC showed that there were significant differences between patients with sickle cell anemia and healthy people, in children only. While in adults there are no significant differences between patients and healthy. The results also showed that there were significant differences in platelet, and MCV levels between patients with sickle cell anemia and healthy subjects, in adults only, but in children, there were no significant differences between patients with sickle cell anemia and healthy group. In this study, SCA patients with the painful crisis have significant correlations between (visfatin, LDH), and (visfatin, Ferritin).

Abstract: Atherosclerosis is a chronic, complex, inflammatory disease that affects medium to large size arteries (coronary, carotid, peripheral arteries). This condition begins when oxidized low-density lipoproteins build up in the artery intima. This resulted in the production of proinflammatory oxidized lipids by the overlapping endothelial cells. This leads to hardening and narrowing of blood vessels. Aim of study: To find association between obesity with occurrence of atherosclerosis. The study included (100) males with an average age of (40-65), and they were divided as follows: (60) patients groups suffering from atherosclerosis were divided into two groups, depending on the body mass index (BMI): ● The obese group (BMI = 25 ≥ 30) included 30 male subjects. ● The normal weight group, BMI = 18.5-24.9, included 30 male subjects. (40) control groups were divided into two groups, depending on the BMI ● The obese group (BMI = 25 ≥ 30) included 20 male subjects. ● The normal weight group, BMI = 18.5-24.9, included 20 male subjects. This study was conducted from November 2022 to May 2023. The samples were taken from the Karbala Center for Cardiac diseases and surgery, where (5ml) blood samples were taken from patients and control, and the biochemical and physiological tests were conducted on them (adiponectin, leptin, plasminogen activator inhibitor-1 (PAI-1), malondialdehyde (MDA), total antioxidant capacity (T-AIC), troponin I, C-reactive protein (CRP), lipid profile). The result of this study found a highly significant decrease (p≤0.001) in concentration of adiponectin in (atherosclerosis obese, atherosclerosis normal, control obese) as compared to control normal. While highly significant increase (p≤0.001) in concentration of leptin in (atherosclerosis obese, atherosclerosis normal, control obese) as compared to control normal. A highly significant increase XIV (p≤0.001) in concentration of PAI-1 in (atherosclerosis obese, atherosclerosis normal, control obese) as compared to control normal. While highly significant increase (p≤0.001) in concentration of MDA in (atherosclerosis obese, atherosclerosis normal, control obese) as compared to control normal. A highly significant decrease (p≤0.001) in concentration of T-AOC in (atherosclerosis obese, atherosclerosis normal, control obese) as compared to control normal. A highly significant increase (p≤0.001) in concentration of troponin I in atherosclerosis groups as compared to control groups. Found a highly significant increase (p≤0.004) in the concentration of CRP in (atherosclerosis obese, atherosclerosis normal, and control obese) as compared to control normal. A significant increase (p≤0.001), (p≤0.005), (p≤0.021), (p≤0.001) in the concentration of total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL), very low density lipoprotein (VLDL), respectively in atherosclerosis obese as compared to (atherosclerosis normal, control obese, control normal). Also a highly significant decrease (p≤0.001) in the concentration of high density lipoprotein (HDL) in ( atherosclerosis obese, atherosclerosis normal, control obese) as compared to control normal. This study clarified several relationships among parameters in the atherosclerosis obese group. There was a significant negative correlation between adiponectin and MDA. It observes significant positive correlation between BMI and both TG and VLDL. In addition to the positive correlation between TC and LDL. It shows positive correlation between TG and both VLDL, MDA. This study observed several relationships among parameters in the atherosclerosis normal group. There was a significant negative correlation between BMI and T-AOC. It shows a negative correlation between TC and HDL. It shows positive correlation between TG and VLDL. This study observed several relationships among parameters in obese group. There was a significant positive correlation between BMI and PAI-1. In addition, XV negative correlation between BMI and HDL, positive correlation between TC and LDL, positive correlation between TG and VLDL. Conclusion: there is association between (adiponectin, leptin, PAI-1) in the development of atherosclerosis in obese patients. Increased MDA concentration indicates that the rate of oxidative stress is high. Decrease in concentration of T-AOC in all obese groups. Leptin ,which controls metabolism, was found to be increasing, which may be a sign that the metabolic issue in obese individuals in this study.

Abstract: This study aimed to identify the most common bacterial isolation that were correlated with patients suffering from prostate cancer, study antibiotic susceptibility pattern. During the period extending between (November 2021 to April 2022) from the Imam Hussain Center for Oncological and Hematological Diseases Directorate, urine samples were obtained from so suspected patients suffering from prostate cancer, their age were ranged from 51 to 80 years. Collected 50 urine sample. The Vitek 2 system was the method that used for identification of bacterial species. The results showed that out of 50 urine samples, (58%) had non-significant bacteria growth, (42%) were found with significant bacterial growth. The result of bacterial isolates was (81%) of Escherichia coli, (14.3%) of Enterobacter cloacae and (4.7%) of Klebsiella pneumoniae. The most growth of E.coli isolates was within 8-10 Gleason score group (56%), this indicates that this category (Gs 8-10) is a risk factor and an encouragement for the growth of E. coli in prostate patients. The results of the present study showed that E. coli and Enterococcus are present in large number relatively in the urine of subjects with prostate cancer.Increased levels of these two types of bacteria in urine may indicate that a significant degree of inflammation occurs in those with prostate cancer.Current study showed that Gleason score (8-10) with 25 patients (50%); and documented increasing prostate specific antigen in age group (70-79) years. also, the results revealed there were 17 (34%) patients within the overweight group and severe Gleason scores (8-10) that may indicate that being overweight is a risk factor for the severity of prostate cancer. Vitek 2 compact system was used to check for antibiotic sensitivity among all of the identified isolates. The extracted data show that out of 46 antibiotics tested, E. cloacae isolates (A10: 33, A18 and A39: 11) were resistant to antibiotics and (A10: 5, A18 and A39: 21) sensitive to antibiotics with different percentage, K. pneumoniae was resistant to 3 antibiotics and sensitive to 22 antibiotics and E. coli isolates was resistant to 29 antibiotics, intermediate to 3 antibiotics and sensitive to 32 antibiotics with different percentage among isolates.

Abstract: Summary
Staph. aureus is a normal flora and opportunistic pathogen. It is the most
common in skin and soft tissue. This microbe can cause more diseases, for
example: burn inflammation and tonsillitis through the production of virulence
factors that are acquired by some plamidic virulence genes. The main research
objective explored the prevalence of (edin-c, sej and etb genes) in each patients
and healthy and its relationship with the development of infection in different
clinical sources.
This study was conducted in the Laboratory of Department of the
Pathological analysis, Faculty of Science, University of Thi-Qar from October
2019 - March 2020, A total of 640 samples were collected from patients and
healthy human. The research samples include 170 of patients suffered from burns
in the department of burns in Al-Hussein Teaching Hospital, also 150 samples of
patients were clinically diagnosed as having tonsillitis infections in the “E.N.T”
department in the AL Habboby Hospital while human healthy samples were 320
which collected from different parts of the body. These samples were cultured on
routine bacterial culture media which include: blood agar, MacConkey agar,
Mannitol Salt and DNAse, tested by Gram's stain, catalase test, coagulase test and
confirmed through API staph 20(BioMereux, France). Antibiotic susceptibility test
was performed by the modified Kirby-Bauer disc diffusion method according to
CLSI guidelines, while molecular diagnosis was depended on 16srRNA gene to
indicate Staph.aureus.
The data were statistically analyzed using the Chi-square test in SPSS
version 20; P-value ≤ 0.05 at 95%( The level of confidence) was considered for
statistical association.
According to the cultural, morphological, and biochemical characteristics,
Prevalence of Staph. aureus were 17.6% (n=30), 13.3% (n=20), and 5.94% (n=19)
II
to Burn patients, tonsillitis patients and healthy human. Production of some
virulence factors by Staph. aureus were been investigated. The results showed its
ability to produce hemolysin toxins, protease enzyme, and urease enzyme that
contributed to escape from the host immune response.
The study included testing all isolates against 12 antibiotics by the disc
diffusion method. The results revealed that Staph. aureus isolates were resistant to
Penicillin-G 100% in tonsillitis and healthy human isolates, but 90% in burn
isolates. Vancomycin resistance in tonsillar isolates was 15 % and 3.3 % in burn
isolates, but 0% in healthy humans. The outcomes of the study indicated that
resistance of Staph. aureus to Ciprofloxacin was 23.3% and 20% in burn,
tonsillitis patients, respectively, but 15.78 % in healthy humans. This study has
registered Sequential proportions related to Azithromycin. These were 50% for
burn patients and 60% for tonsillitis patients, but 42.10 % in healthy humans.
Clindamycin resistance in burn and tonsillitis were 73.3% and 75%, but in healthy
human were 5.26%, respectively. Nitrofurantoin 's resistance showed 13.3% to
burn and 10% to isolates of tonsillitis. Levofloxacin displayed the same resistance
levels(13.3% & 10%, respectively) to both clinical sites, but it was compared with
healthy humans, was zero %, and 10.52% to Levofloxacin, Nitrofurantoin,
respectively. The current study showed a noticeable rise in Erythromycin
resistance, which reached to 80% and 57% for both tonsillitis and burn isolates,
respectively, but 42.10% in healthy human. The antibiotic resistance values for
tonsillitis isolates of Ofloxacin and Gentamycin were the same at 15%, although
both antibiotics were recorded at 13 % and 10 % for burn isolates respectively,
whereas for healthy humans the levels of Ofloxacin and Gentamycin were 10.52
%.It was observed in study that the rate of antibiotic resistance to tetracycline 30%
and 23.3% for tonsillitis and burn isolates, respectively but in healthy humans was
36.84%. Close ranges of doxycycline resistance obtained at 20 % and 16.7 % for
tonsillitis and burn isolates, respectively, but were 21.05 % for healthy humans.
III
The results of phenotypic diagnostic were confirmed by polymerase chain
reaction (PCR) technique employing 16SrRNA gene that showed only 48\50
certain bacterial isolates of this microbe from patients while in healthy humans
were in line with phenotypic diagnostic (19\19), but results of molecular screening
of prevalence edin-c, sej and etb genes in burn patients had showed 48.27%,
62.06% and 62.06% respectively while in tonsillitis patients was 42.10%, 73.68%
and 47.36%. comparing to healthy humans, there were no results to determine
edin-c, sej except etb (21.05%).
