Abstract:
يعد مرض اللاهودجكن اللمفاوي مجموعة من سرطانات الخلايا اللمفاوية والذي تتطورفيه الخلايا المفاوية بطريقة خاطئه مؤدية الى اضطرابات مناعية .تم خلال الدراسة الحالية تقييم بعض المؤشرات المناعية في امصال (46) عراقيا من مرضى اللاهودجكن اللمفاوية ومقارنتها مع (46) فردا مضاهيا من الاصحاء ظاهريا .اجريت الدراسة في المركز الوطني لبحوث وعلاج امراض الدم وفي قسم علوم الحياة في الجامعة المستنصريه منذ كانون الثاني الى حزيران من العام 2013. هدفت الدراسة لتقييم مستويات الانترليوكين 10 والاديبونكتين واضداد (EBV) في امصال مرضى اللاهودجكن اللمفاوية ودراسة اثر هذه المتغيرات في الامراضية والتنبؤ بسير المرض وكيف يؤثر بعضها في البعض .كان 31 (67%) مريضا (≥ 50) سنة و15 (33%) منهم باعمار(<50) سنة . وكان متوسط اعمار المرضى (52.74متوسط± 2.18) اذ تراوحت اعمارهم بين (17 - 80) سنة وقد بينت نتائج الدراسة ان الذكور هم اكثر عرضه للاصابة بالمرض مقارنة بالاناث وبنسبة 1 : 1.7 . كما تبين من الدراسة هنالك فروقات معنوية (0.05 > p) بين المجموعتين العمريتين (50≤ و50≥) فضلا عن الفروقات كانت معنويه(0.05 > p) بينهما في مستويات الاضداد (EBV IgM) في الوقت الذي لم تكن الفروق معنويه (0.05 < p) لاضداد (EBV IgG) . تبين كذلك ان هنالك فروقات في مستويات الاديبونكتين المصلية (0.05 > p) بين المجموعتين العمريتين في ما لم تكن الفروقات معنويه (0.05 < p) في مستويات الانترليوكين 10 بين المجوعتين العمريتين . عند تقسيم المرضى تبعا للجنس 29 ذكر و17 انثى , بينت النتائج ان هناك فروقات معنوية في اعداد خلايا الدم البيض بين الجنسين (17.32*109/لتر ± 6.87 ذكور , 5.80*109/لتر± 0,85 اناث) (0.05 > p).وبشان مستويات اضداد EBV (IgM,IgG) لم تكن هنالك فروقات معنويه (0.05 < p) بين كلا الجنسين , كما لم تكن هنالك فروقات معنويه (0.05 < p) في مستويات الاديبونكتين . فيما كانت الفروقات ذات دلاله معنويه في مستويات الانترليوكين 10 (23.77 بيكو غرام / مل ±6.58ذكور , 12.06 بيكوغرام/مل ±3.77 اناث ) (0.05 > وبمقارنة المرضى بالاصحاء كانت هناك فروقات معنوية (0.05 > p) في اعداد كريات الدم البيضاء فما لم تكن الفروقات معنوية (0.05 < p) بالنسبة EBV (IgM,IgG) .اما مستويات الاديبونكتين المصلية فقد انخفضت معنويا في امصال مرضى اللاهودجكن المفاوية (وان بقيت مستوياتها ضمن الحدود الطبيعيه ) بالمقارنة مع الاحصاء (0.05 > p).سجلت النتائج ارتفاعا معنويا (0.05 > p) في مستويات الانترليوكين 10 (18.67بيكو غرام/مل ±4.12 )بالمقارنة مع مستوياته (3.4بيكوغرام /غرام ±0.73) عند مجموعة الاصحاء , كما لوحظ ان هنالك ارتفاعا معنويا (0.05 > p) اخر في امصال اللاهودجكن ذوي المراحل المتقدمة (33.1 بيكو غرام /مل ± 4.5) عند مقارنتهم بالمرضى في المراحل المبكرة (13.4 بيكو غرام /مل ± 2.7) (0.05 > p)وعلى العكس من ذلك انخفضت مستويات الاديبونكتين معنويا (0.05 > p)في المراحل المتقدمة من المرض (4.53مايكروغرام /مل ±1.4 ) مقارنة بالمراحل المبكرة (6.79مايكرو غرام /مل ±1.1). بينت نتائج التحليل الاحصائي انه ليس هنالك ثمة علاقة معنويه (0.05 < p)بين الانترليوكين 10 والاديبونكتين عند مرضى اللاهودجكن اللمفاوي في جميع مراحلة . خلصت الدراسة بالاستنتاج ان حالات اللاهودجكن المدروسة اكثر حدوثا في فئات عمرية فوق الخمسين سنة بالمقارنة مع من هم تحت ال 50 سنة وترافقهزايادة في مستويات الانترليوكين 10 مع تقدم عمر المريض وارتباطها كذلك مع شدة المرض مما قد يعكس القيمة التنبؤية لهذا السياتوكين . فيما ارتبط انخفاض مستويات الاديبونكتين مع ارتفاع خطورة الاصابة بالمرض ومع اشتداد المرض . يرتبط الانخفاض في مستويات الاديبونكتين عكسيا مع مؤشر كتلة الجسم (BMI). سجل خلال الدراسة انخفاض مستويات (EBV IgM) في المرضى باستخدام تقنية الايلايزا يعكس الحاجة لاستخدام تقنيات اخرى للكشف عن الفايروس المختبىئ . | Non hodgkin's lymphoma (NHL) is a group of cancers of the lymphocytes. In NHL, the lymphocytes develop in a wrong way led to immune disorder. The present study was carried out to evaluate some immunological parameters in the sera of (46) NHL Iraqi patients , In addition to control group involving (46) matched apparently healthy subjects. This study was done in the National center of hematology & Department of biology at AL - Mustansiyria University since January to June 2013.The aim of this study is to estimate the levels of Interleukin 10, Adiponectin and EBV infection in sera of patients with NHL and to study the impact of these parameters on the pathogenesis and predictive of the disease and how they correlated.There were 31 out of 46 (67%) patients with age group ≥50 years and 15 out of 46(33%) patients with age group <50 years. The mean age of NHL patients was (52.74) years ranging from 17 to 80 years. The patients were divided into two groups according to the age(≥50years and <50 years)patients .There was statistically significant difference (p<0.05) in White blood cells count between age groups . The mean of adiponectin serum level was significantly differ (p<0.05) between the two age groups.The mean of IL - 10 serum levels did not show a significant difference between the two age groups (p>0.05).The mean of adiponectin serum level in NHL patients wasn't significantly (P>0.50) differ between male and female while the IL - 10 level in serum was significantly different (23.77pg/ml ± 6.58 male, 12.06 pg/ml ± 3.77 female) (P<0.05).When the patients of NHL compared with control, the mean of white blood cell count was significantly different ( p<0.05) also there was non - significant differences(P>0.05) in mean of EBV - IgG & IgM among NHL patients and control group. The mean level of adiponectin serum was a statistically decreased in patients of NHL as compared with control subjects(p<0.05), but still these levels are within normal range, furthermore The levels of interleukin 10 were significantly increased in patients with NHL compared with healthy subjects (18.67 ± 4.12 pg/ml vs. 3.4 ± 0.73pg/ml) (p<0.05). Higher increasing of IL - 10 levels in the advanced stage patients of NHL was noticed compared with early stages (33.1 ± 4.5 pg/ml vs. 13.4 ± 2.7 pg/ml) (P<0.05). Otherwise the serum levels of adiponectin in aggressive stage of NHL patients was decreased compared to the early stages of that disease (4.53 ± 1.4 µg/ml vs. 6.79 ± 1.1 µg/ml ) (P<0.05). There was no significant negative correlation between IL - 10 and adiponectin in all NHL patients (p>0.05) also there was no significant correlation between IL - 10 and adiponectin in patients with advanced stage (p>0.05). In conclusion, NHL was noticed in the highest ages and it might indicates the increasing rates of NHL among older population. in addition increasing level of IL10 is associated with increasing age range and also correlated positively with aggressive NHL cases that might indicates the predictive pivotal role of this cytokine . Moreover, Lower adiponectin levels are associated with an increased risk of NHL. Adiponectin is inversely correlated with Body mass index and also it was decreased in aggressive cases compared to early stages. The low percentage of EBV IgM in NHL patients by using Elisa technique might indicates the Necessity for using other advanced technique to detect EBV infection either an action or silent form
