Abstract:
دراسة ضابطة قبلية وبعدية (تصميم شبه تجريبي) في مستشفى بابل للنسائية والاطفال على التدوين التمريضي والطبي. هذه الدراسة بدات من (29/9/2016) واستمرت الى (11/5/2017). وتهدف الى التعريف بخطوات تطبيق السجل الطبي الالكتروني في وحدة العناية المركزة للاطفال في المستشفى المبحوثة وحساب الوقت وتقيم اكتمال التسجيل قبل وبعد تطبيق السجل الالكتروني. وقياس تقبل العاملين في الوحدة للنظام حسب نموذج الدراسة المقترح باعتماد نموذجtechnology acceptance model 3 . وتم انتخاب عينة احتمالية ملاءمة من المستشفى المبحوثة، طبقت على (120) طفل راقد في الوحدة قبل تطبيق النظام (60) مريض وبعد التطبيق (60) مريض، حيث تمت مراجعة سجلاتهم. وعينة ثانية متعمدة (غير احتمالية) عددها (30) من الفريق الطبي والتمريضي في الوحدة بعد 3 اشهر من تطبيق النظام. اما ادوات الدراسة فكانت ساعة توقيت مع ورقة جداول بيانات اكسيل لجمع البيانات الخاصة بحساب الوقت وتقييم اكتمال التدوين، واستبانة الكترونية تم تصميمها من قبل الباحث بعد المراجعة المستفيضة للدراسات السابقة المعنية، وجمعت البيانات من خلال ارسال رابط الاستبيان الى المشاركين عبر (البريد الالكتروني او وسائل التواصل الاجتماعية).تم التاكد من مصداقية الاستبانة من خلال دراسة تجريبية. وتم استخدام الاحصاءات الوصفية والاحصاءات الاستدلالية لتحليل النتائج. اظهرت الدراسة ان وقت التوثيق اسرع باستخدام السجل الالكتروني (10.25 min 0.38 sc vs 23.9 min 0.43sc ; p<0.001). كما تحسن اكتمال تدوين السجلات الالكترونية بشكل ملحوظ بعد استخدام النظام(mean 226.65 14.815 vs. 218.13 10.835; p<0.001) ، كما وبينت نتائج تحليل الاستبانة يوجد لدى المستخدمين سلوكا ايجابيا اتجاه استخدام النظام. وبهذا يكون المشروع قد نجح تطبيقه. وتوصي الدراسة باعتماد الية لتحفيز الموظفين ماديا ومعنويا على استخدام السجلات الالكتروني، لزيادة الكفاءة في العمل | A controlled before - after study (Quasi - experimental design) has been conducted in Babylon Maternity and Children Hospital on nursing and medical documentation. This study started from 29th Sep 2016 to 11th May 2017. It aims to identify the implementation steps of the electronic record in the pediatric intensive care unit in researched hospital, the time calculating with completeness evaluating of documentation in the before - after period, and evaluate the unit workers' acceptance of the system according to the proposed model, which was based on the technology acceptance model3.A convenience (probability) sample selected from researched hospital conducted on (N=120) pediatric patients admitted to the unit before application (n=60) and after (n=60). And a purposive (non - probability) sample selected on (n=30) unit staff after 3 months of application. The tools were stopwatch with excel spreadsheets designed to gather data, and an electronic questionnaire which was designed by the researcher, the data were gathered through the questionnaire link sent via (E - mail, social media) to the participants.Reliability and validity of the electronic questionnaire were verified through a pilot study. The descriptive analysis and inferential analysis were used to analyze the results.The results were the documentation time was faster using electronic recording, (10.25min0.38sc vs 23.9min 0.43sc; p<0.001). And documentation completeness was enhanced significantly after application ( mean 226.65 14.815 vs. 218.13 10.835; p<0.001). Also, the users have a positive attitude toward the system use.This study recommends by adopting of monetary and ethical recompenses, like credentials, to encourage staff to use the system.
