المستخلص: This study was designed to fined out the prevalence of anti - HCV in thalassemics and diabetic patients and HCV viremia in anti - HCV positive patients, to delineate the role of TH1 cytokines in HCV infection, and to fined out a possible diabetogenic role for HCV infectionThis study was carried out in Erbil province for the period Feb 2006 to March 2007. Different diagnostic test approaches are used like 3rd generation ELISA, RT - PCR to detect HCV infection and ELISA for cytokine determination. The screened patients and control group including : (347) β - thalassemics, (786) diabetic patient, and (465) as control population were subjected to above tests for the purpose of clarifying the sensitivity and specificity of ELISA vs PCR in risk and control groups moreover, to delineate the role of TH1 cytokines in HCV infection.The results can be briefly summarized as following : Anti - HCV anti body were detected among (28.24%) (5.97%), and (0.43). of thalassemics, diabetics, and control population. RT - PCR testing revealed (88.77%), (68%) positivity among anti - HCV positive thalassemics, and diabetics cases screened by ELISA test.Cytokine levels in thalassemics for TNF - α, IFN - γ, and IL - 2 were significantly higher in PCR - confirmed HCV cases than in HCV - negative thalassemics (P < 2.017E - 17), (P< 1.67E - 5), and (P< 6.048E - 8) respectively. A statistically higher significant levels of TNF - α, IFN - γ, and IL - 2 demonstrated also in thalassemic patients (PCR - confirmed HCV thalassemics and, HCV - negative thalassemics ) than in healthy controls In diabetic patients revealed only high significant association between PCR confirmed HCV infection and increased level of TNF - α (P < 1.19E - 10).Statistically significant high level of TNF - α only was detected among diabetic patients (PCR - confirmed HCV diabetics and, HCV - negative diabetics) than healthy control group (P < 1.19 E - 10).In PCR confirmed - HCV thalassemic patients (4.6%) were found to be diabetic in comparison with (0.9%) in HCV - negative thalassemic group.This study clearly demonstrates the highly prevalence of HCV - infection among thalassemic patients and to a lesser extent in diabetics.The elevation of TH1 cytokines (TNF - α, IFN - γ, and IL - 2) in thalassemics may contribute to cell mediated immunity and may be related to other infections not just HCV as a reason of frequent blood transfusion. The Study also confirm that the
