Abstract: Toxoplasmosis is the general term for infection and disease in human and animal caused by a parasite called Toxoplasma gondii, this disease is prevalent among different categories of people , Heart disease patients are one of them. Because of the dormancy status of T. gondii, there is ambiguity and variation in the immunity of patients. This study aimed to investigate the immune status of heart disease patients infected with toxoplasmosis, to detect the prospective relationship between heart disease patients and T. gondii, and investigate the relationship between toxoplasmosis and risk factors which included gender, age, blood group, contact with cats, and habitation.One hundred fifty samples of blood had been collected at period from August 2016 till November 2016 from patients with heart disease, they lying or visit Heart Medical Centre in Thi - Qar province. According to gender, the patients divide in to (81 males and 69 females) with age ranging 18 to 82 years. Also one hundred samples of blood had been collected from apparently healthy persons as negative control ,dividing according to gender in to ( 55 males and 45 females) with age ranging 19 to 56 years . All samples of both subjects had been tested by ELISA technique to detect anti - Toxoplasma Abs (IgG & IgM) . While the positive samples with toxoplasmosis were tested to detect the level of interleukins (IL - 10, IL - 12 and IL - 17).The overall of seropositivity of anti - Toxoplasma Abs that scored by ELISA in the current study was 44% in heart disease patients are higher than those in apparently healthy controls 19%, and there was a significant difference between them. There is a significant difference between male and female, in the seropositivity of anti - Toxoplasma Abs that scored in controls, in male was higher than those in female. Presence of antiSummary toxoplasmosis Abs in all types of heart disease patients with significant difference among them, and the highest percentage of anti - toxoplasmosis Abs in Mitral stenosis( MS) was 55.8% , and the lower value was 30.3% in Myocardiopathy.There are significant differences between the age groups and the percentage of anti - Toxoplasma Abs in heart disease patients with highest value was 63.2% at age group of 61 - 75 years, and the lowest one was 26.3% at age group of 15 - 30 years. While no significant differences between the age groups and toxoplasmosis in apparently healthy controls with highest value was 19.5% at age group of 31 - 45 years, and the lowest one was 8.6% at age group of 15 - 30 years.The results demonstrated that seroprevalence of toxoplasmosis was significantly associated with close contact with cats. The percentage of seroprevalence anti - Toxoplasma Abs in heart disease patients and controls inhabited rural area was 52.5% and 32% are higher than those inhabited urban area was 34.3% and 6% respectively. Some blood parameters were estimated of both subjects infected and non - infected with toxoplasmosis. There was high significant difference at p≤0.05 observed in mean ratio of eosinophil’s number between infected and not infected subjects with toxoplasmosis in both subjects. While there was no significant differences in mean ratio of total count of WBCs. Moreover, mean of lymphocytes number recorded significant differences betweeninfected and non - infected heart disease patients only. The blood group phenotypes of the infected heart disease patients and controls highlight the role in toxoplasmosis infection and the AB blood group characterized by the highest significant percentage of infection. The results of interleukins appeared high mean levels of IL - 12 82.85 pg/ml in heart disease patients infected with toxoplasmosis when compared with control 60.97 pg/ml, and there was significant difference between them. While there was no significant difference in IL - 10 and IL - 17 in both patients and controls. The results also showed decreased level of IL - 17 in patients with heart disease. The increased level of IL - 12 in patient with heart disease infected with toxoplasmosis indicate to the activity of cell mediated immunity in those patients and may be lead to provoke an inflammatory status in the heart valves and muscle then worse heart disease to be sever and need complicated treatments
