Abstract:
Markers have been described in type 1 diabetes mellitus (T1DM), There is a number of specific and non specific antigens have been identified. The major autoantigens involved in the destructive process of beta - cells leading to the development of type 1 diabetes are insulin hormone, glutamic acid decarboxylase (GAD), tyrosine phosphatase enzyme or Insulinoma - associated Antigen - 2(IA - 2).This study was conducted to find the relationship between antibodies for this antigens (insulin, GAD and IA - 2) and T1DM which could be used for the early detection of T1DM in normal Iraqi population. To study the importance of anti - thyroid peroxidase (anti - TPO) as a marker for autoimmune thyroid disease (ATD) in T1DM patients, and to find the relationship between T1DM and ATD.The study was carried out on 50 blood samples of men and 30 blood samples of women with age ranged from (20 - 60 years old), they were divided in to three groups : 1. Group 1 (20 men and 10 women) whom have fasting plasma glucose (FPG) above 180 mg/dL.2. Group 2 (20 men and 10 women) whom have FPG ranged from 120 - 180 mg/dL.3. Group 3 (10 men and 10 women) whom have FPG below 120 mg/dL.Blood samples were collected from all subjects, FPG and glycated haemoglobin (HbA1c) were measured. The levels of insulin, anti - insulin, anti - GAD, anti - IA - 2 and anti - TPO antibodies were measured in the serum.The statistical analysis results showed no significant difference in the presence of antibodies, HbA1c and FPG between men and women groups, inspite of some simple differences between the two groups.A significant (p<0.05) elevation in the level of FPG and HbA1c was observed in diabetic patients group compared withnon diabetic group. Significant (p<0.05) decrease in the level of insulin of T1DM patients was noticed compared to non diabetic group. Significant (p<0.05) elevation in the level of (anti - insulin, anti - TPO) in T1DM patients compared with non diabetic group. Elevation in the level of (anti - GAD, anti - IA - 2) in the T1DM patients compared with non diabetic group.The results also showed that no positive results for (anti - insulin, anti - TPO) present in the non diabetic group. One positive result for anti - GAD and one positive result for anti - IA - 2 present in the non diabetic group, which indicate the importance of anti - GAD and anti - IA - 2 antibodies ssay in normal population that could be used as early detection of T1DM.
