Abstract:
سرطان الدم هو مجموعة من الامراض التي تتميز بزيادة في عدد الخلايا البيضاء في الدم ونخاع العظام ويحدث بسبب تكاثر غير مسيطر عليه لخلايا سرطانية غير ناضجة .العامل المثبط لسرطان الدم (LIF) قد تلعب دورا هاما، الى جانب (IL6) و(G - CSF)، في تنظيم الخلايا الجذعية المبكرة للدم .وكذلك اللبتين ((Leptin ذات تاثير في تمايز وتكاثر الخلايا المكونة للدم.العامل المثبط لسرطان الدم (LIF),عضو في عائلة (IL6) الكبيرة. واسمها مشتق من قدرتها على الحث على التمايز النهائي بين خلايا سرطان الدم النقوي ومنع نمو خلايا سرطان الدم. انترلوكين 6يمتلك تاثير تنشيطي وقمعي بنفس القدر.ويزداد مستوى IL6 في مرضى سرطان الدم النقاوي.اما اللبتين يفرز من الخلايا الدهنية (adipocytes) التي تشغل ما يقرب كل تجويف نخاع العظم في الانسان وتحفز النمو النقوي والمحمر. ولذلك اللبتين ومستقبلات اللبتين قد تلعب دورا حيويا في السيطرة على التمديد والتمايز بين الخلايا البدائية المكونة للدم خلال تفاعل النضير الصماوي في نخاع العظام والمعنية في تطوير الاورام الخبيثة للدم. الاهداف : الاهداف الرئيسية لهذه الدراسة هي تقييم مستوى العامل المثبط لسرطان الدم (LIF) ،انترلوكين6(IL6) واللبتين في المصل عند المرضى العراقيين (مدينة بابل) المشخصين حديثا بسرطان الدم النقوي ومقارنتها مع الاشخاص الغير المصابين بالمرض المذكور. اضافة الى بعض الفحوصات الدموية مثل قياس الهيموغلوبين وحجم الخلية المضغوط (PCV)، عدد الصفيحات الدموية,عدد الكريات الدم البيضاء وفيلم الدم وكذلك نسبة الدهون.وثمة هدف اخر تحديد الترابط بين اللبتين مع مؤشر كتلة الجسم .المواد وطرق العمل : هذه الدراسة هي دراسة مقارنة وشملت هذه الدراسة 48 مرضى سرطان الدم النقوي تم تشخيصهم حديثا (30مريض مصابين بابيضاض الدم النقوي الحاد ، ومتوسط عمر 44.43 ±10.75 سنة) و(18 مريض مصابين بابيضاض الدم النقوي المزمن ومتوسط العمر 59.27 ±6.01 سنة). بالاضافة الى 48شخص سيطرة ادرجت فى هذا الموضوع . تم جمع عينات المرضى ومجموعة السيطرة خلال الفترة من ايلول/2015 الى نيسان 2016. لقد تم تقييم مستوى العوامل المثبطة لسرطان الدم (LIF) ،انترلوكين6(IL6) واللبتين في المصل باستخدام مقايسة الممتز المناعي المرتبط بالانزيم (ELISA) من خلال استخدام Human Elisa kit. الفحوصات الدموية تمت باستخدام جهاز(.(auto analyzer ونسبة الدهون باستخدام جهاز Reflotron.النتائج : لقد بينت النتائج وجود زيادة احصائية معنوية بمتوسط مستويات العامل المثبط لسرطان الدم (LIF) ،والانترلوكين6(IL6) (P<0.05). اما متوسط اللبتين في مجموعة المرضى كان منخفض معنويا عنه في مجموعة السيطرة (P<0.05). من ناحية اخرى تبين عدم وجود فروقات معنوية بمتوسط مستويات العوامل المثبطة لسرطان الدم (LIF) ،انترلوكين6(IL6)واللبتين بين مرضى بسرطان الدم النقوي الحاد والمزمن (P <0.05). وكان هناك ارتباط معنوي بين متوسط مستوى اللبتين ومؤشر كتلة الجسم (P<0.05).الاستنتاجات : ان مستوى العامل المثبط لسرطان الدم (LIF) ،والانترلوكين6(IL6)في المرضى اعلى منه في مجموعة السيطرة، بينما مستوى اللبتين في المرضى اقل منه في مجموعة السيطرة كما انه توجد علاقة معنوية عالية بين مستوى اللبتين ومؤشر كتلة الجسم | Leukemias are a group of diseases described by augmented numbers of white cells in the blood and bone marrow as a result of unregulated clonal proliferation of immature malignant cells.Leukemia inhibitory factor (LIF) may play an important role, along with Interleukin - 6 (IL - 6) and granulocyte colony stimulating factor (G - CSF), in the regulation of early hematopoietic stem cells. Leptin has been implicated in the differentiation and proliferation of hematopotietic cells.Leukemia inhibitory factor is a member of the interleukin - 6 cytokine superfamily. Its name was derived from its ability to induce the terminal differentiation of myeloid leukemia cells in order to prevent the growth of leukemia cells.Interleukin 6 shows the ability to have equal stimulatory and suppressive effect. IL - 6 level is increased in patients with myeloid leukemia.leptin is secreted from local adipocytes, which occupy nearly all of the marrow cavity in human, and stimulates myeloid and erythroid growth. Therefore, leptin and leptin receptors may play a vital role in the control of the extension and differentiation of primitive hematopoietic cells during paracrine interaction in the bone marrow and concerned in the development of hematopoietic malignancies. Aim of the study : The aim of this study is to evaluate serum level of LIF, IL - 6 and leptin in Iraqi(Babylon province) patients newly diagnosed with myeloid leukemia patients and normal controls. In addition to some hematological parameters like Hb, PCV, Platelet, WBCs count and blood film as well as Lipid profile assay . Another objective is to determine the correlation of Leptin with body mass index .Subjects and Methods : This case control study included 48 patients with a newly diagnosed myeloid leukemia (30 patients with AML and mean age 44.43 ±10.75 years) and (18 patients with CML and mean age 59.27 ±6.01 years). Another 48 healthy controls were included.The samples collected during the period from September/2015 to April /2016. Serum LIF,IL6 and leptin levels are estimated by enzyme linked immunosorbant assay (ELISA) using Human ELISA Kit while hematological investigation were done by auto analyzer. Lipid profile assay was performed by Reflotron device .Blood film examined by expert hematology. Results : The results showed that the LIF and IL6 levels in the patient group was significantly higher than control group (P <0.05) while leptin level in the patient group was significantly lower than control group level, however, LIF, IL - 6 and leptin levels shows insignificant difference between AML and CML (P >0.05). There is a significant correlation between mean leptin level and mean BMI (P <0.05).Conclusions : The LIF and IL6 levels in patients with newly diagnosed myeloid leukemia is higher than in controls, while Leptin level in the patient group is significantly lower than control group level and there is a high significant correlation between Leptin and BMI.
